Factors associated with preterm labour and changes in the cervix before labour in twin pregnancy

Abstract: Summary. A total of 132 twin pregnancies in black African women were studied prospectively after 30 weeks gestation. Delivery occurred before 37 weeks in 32%. There was a trend (0.1 > P > 0.05) towards a higher preterm delivery rate in nullipara (57%), in women under the age of 20 years (60%) and in those with a height/weight ratio of >2.5 (50%). The cervix was assessed with a score based on the length of the canal minus the dilatation of the internal os. In both term and preterm labour there was a significan… Show more

“…In women suffering from cervical incompetence, the relation between 3D-and 2D-results was not altered. Aside from 2D metric changes, an alteration in the shape of the cervix is known to be associated with cervical incompetence [6]. Different authors stress the important difference between symptoms of cervical incompetence and preterm labor; the two can overlap because cervical dilatation caused by membrane herniation can induce uterine contractions.…”

“…In addition to transabdominal ultrasound, manual pelvic examination was performed to evaluate cervical length and effacement, consistency and position as well as position of the presenting fetal part. Sonographic data was compared to the pelvic score established by Bishop [6][7][8]. Values were expressed as mean B SD.…”

“…Cervical score was subsequently calculated as length minus dilation. 19 Selected demographic information, behaviors, medical history, and antenatal medical events were obtained through telephone interview. These data were supplemented with laboratory results of antenatal sexually transmitted disease (STD) cultures and, for a preterm case/term control subset of the cohort, medical chart entries for antenatal STD and BV diagnoses.…”

“…1995). Houlton et al (1982) described a cervical score for twin pregnancies, which incorporated solely cervical length and dilatation, thus removing the more subjective criteria in the Bishop ' s score (cervical consistency and position, fetal station). While Houlton ' s cervical score appears slightly better at 28 weeks, the Bishop ' s score is a better predictor of preterm delivery at 24 weeks (Newman et al 2008).…”
