Factors Associated With Patient Satisfaction Measured Using an Iterative Scale

Abstract: Background and Objectives: Patient experience measures tend to have notable ceiling effects that make it difficult to learn from gradations of satisfaction to improve care. This study tested 2 different iterative satisfaction measures after a musculoskeletal specialty care visit in the hope that they might have less ceiling effect. We measured floor effects, ceilings effects, skewness, and kurtosis of both questionnaires. We also assessed patient factors independently associated with the questionnaires and the… Show more

“…Skewness and kurtosis are rough indicators of a normal distribution of values: skewness is an index of the symmetry of a distribution, while kurtosis is a measure used to describe the tailedness ( 35 ). Symmetric distributions have a skewness value of 0 and a kurtosis value of 3 ( 36 ). If the skewness value is less than 3 and the kurtosis value is less than 10 ( 37 ), it is regarded as basically acceptable that the sample obeys a normal distribution.…”

“…There are probably some types of empathetic opportunities that merit longer visits, but in general, there are many parts of the visit that can be shorter (such as clinician expertise transfer), and responding to empathetic opportunities does not add much time if there is a corresponding shortening of "teaching" or "explaining" [75]. Going forward, it would be helpful to have a measure of patient experience that provides enough spread in scores to monitor the impact of improvement initiatives [4,57,76]. Preliminary evidence suggests that refining instruments using patient interviews [63], accounting for personal factors such as social desirability bias [4], or using natural language processing to develop quantitative ratings of patient experience [57] can create a broader, more Gaussian distribution of scores.…”

“…Patient satisfaction refers to the degree of consistency between the medical services patients expect and those they receive ( Hooker et al, 2019 ; Ferreira et al, 2023 ). It serves as a vital indicator of a hospital’s fulfillment of its social duties, internal management, and service effectiveness ( Asamrew et al, 2020 ; Versluijs et al, 2023 ). Owing to the increased attention paid to the quality of medical services, patient satisfaction has gained global recognition ( Black et al, 2021 ; Grasso et al, 2021 ).…”
