Several studies have investigated geographical variations in access to renal transplant waiting lists, but none has assessed the impact on these variations of factors at both the patient and geographic levels. The objective of our study was to identify medical and nonmedical factors at both these levels associated with these geographical variations in waiting-list placement in France. We included all incident patients aged 18-80 years in 11 French regions who started dialysis between January 1, 2006, and December 31, 2008. Both a multilevel Cox model with shared frailty and a competing risks model were used for the analyses. At the patient level, old age, comorbidities, diabetic nephropathy, non-autonomous first dialysis, and female gender were the major determinants of a lower probability of being waitlisted. At the regional level, the only factor associated with this probability was an increase in the number of patients on the waiting list from 2005 to 2009. This finding supports a slight but significant impact of a regional organ shortage on waitlisting practices. Our findings demonstrate that patients' age has a major impact on waitlisting practices, even for patients with no comorbidity or disability, whose survival would likely be improved by transplantation compared with dialysis.

Calidad de vida en pacientes trasplantados renales de una institución prestadora de servicios de salud en Medellín, ColombiaQuality of life in transplanted renal that consulted in an institution of health in Medellin, Colombia AbstractIntroduction: Renal transplantation performed on patients with chronic renal disease (CRD) stage five, is an alternative treatment that prolongs survival, reduces morbidity, accelerate social and medical rehabilitation. However it is necessary to carry out studies to evaluate the quality of life of these patients. Objective: Establishing the socio-demographic, clinical and service characteristics of the health insurer that explain the life quality of kidney transplanted patients who consulted a health institution in Medellin in 2016. Materials and methods: Quantitative and transversal survey carried out in 228 patients to whom the SF-36 instrument were applied. There were calculated the statistical tests of Kolmogorov Smirnov for normality, U de Mann Whitney, Kruskal Wallis and the Spearman correlation coefficient. In the multivariate analysis the median regression model was selected. Results: 50% of the patients got 83.3 or less in its life quality score. The multivariate regression model shown significant differences in the time variables after transplantation and readmission to hospital in the last year; by each readmission to hospital, the values of life quality decreased in 3.82 points; according to the time after the transplantation, it was found that going from a state where the patient has a term between 7 and 36 months, to a

Summary Manualized psychotherapy for the optimization of immunosuppressant adherence following kidney transplantation: Results of a feasibility studyObjectives: Adherence can be defined as the extent to which patients follow the instructions for prescribed treatments. Up to 37 % of renal transplant recipients are classified as nonadherent to immunosuppressant medication. Nonadherence, however, is significantly correlated with transplant rejection and loss. The objective of this manual is to optimize adherence to immunosuppressant medication, to improve health literacy, and to raise quality of life among renal transplant recipients. Methods: This article introduces a therapeutic manual and shows the results of a feasibility study. Results: Seven patients were recruited for the feasibility study. Acceptance of the participating patients was clear, and there was no treatment discontinuation. All planned contents could be discussed. In addition to the side effects of immunosuppressants, social support was also discussed as an important topic. Conclusions:The intervention can be implemented following kidney transplantation as a regular and stable offer together with the available resources of nephrological and psychosomatic regular care facilities. It is thus easily transferable into the services of transplant centers.
