Factors associated with delayed and late ART initiation among people living with HIV in BC: results from the engage study

Abstract: We examined correlates of late and delayed initiation of antiretroviral therapy (ART) in British Columbia, Canada. From December 2013 to December 2015 we recruited treatment-naïve people living with HIV who initiated ART within the previous year. 'Late initiation' was defined as CD4 cell count ≤500 cells/µL at ART initiation and 'delayed initiation' as ≥1 year between HIV diagnosis and initiation. Multivariable logistic regression assessed independent correlates of late and delayed initiation. Of 87 participan… Show more

“…The advent of antiretroviral drugs has reduced HIV transmission and mortality worldwide (1). According to the latest report from the World Health Organization (WHO) in 2020, 37.7 million people are living with HIV (PLHIV) worldwide, of which only 73% have access to antiretroviral drugs (2).…”

“…In our review, disparities in receiving ART and ART adherence were found among PLWH in China. The rate of receiving ART among PLWH aged ≥ 45 years is higher than those aged < 45 years, which is different from a report from Canada [72]. Avoidant coping, fear of unintentional disclosure, and stigmatizing social norms may diminish the intention of young PLWH to initiate ART [73].…”

“…A Montreal (Canada) study exploring geographical factors associated with late ART-initiation reported that type of health facility; thus, university medical centres [a 3 rd tier university teaching hospital] versus community facilities [primary tier health facility], were independently associated with early ART-initiation [aOR=2.03] [115]. In addition, two more studies from Canada both demonstrated significant positive associations between patient’s residential status and rapid ART-initiation [71, 101]. In another setting, Ivana & colleagues (2022) in the ECHO Trial involving women from four African nations observed that one’s geographical location had an influence on TTAT.…”
