Factors associated with cytomegalovirus infection in children undergoing allogeneic hematopoietic stem-cell transplantation

Abstract: While preemptive therapy with ganciclovir (GCV) for cytomegalovirus (CMV) infection is used following allogeneic hematopoietic stem-cell transplantation (HSCT), risk factors for CMV infection in children undergoing HSCT are poorly understood. We studied CMV reactivation following allogeneic HSCT by retrospectively analyzing pediatric patients who received allogeneic HSCT and preemptive GCV therapy between 1998 and 2016. The level of viremia requiring preemptive GCV therapy was >1 CMV antigen-positive cells per … Show more

“…However, recently Jaing et al, reported in a population of 290 pediatric patients, which had CMV infection, had statistical significance to develop aGVHD grade III and IV in the univariate analysis and is an independent factor in the multivariate (OR 11 p < 0.001), which provides support for our results, since both studies were performed in the pediatric population [2]. Mexico has an emerging market economy that presents a high incidence of infectious diseases that characterize developing nations, those coexist with chronic diseases, characteristic of developed countries.…”

“…All these risk factors are associated with defective immune surveillance of CMV. Further, the situation of CMV infection in children undergoing allogeneic hematopoietic stem-cell transplantation is more serious than adults with a higher prevalence (13). CMV infection after HSCT mainly manifests as respiratory symptoms and other nonspecific symptoms, which are easily overlooked.…”

“…The frequency of CMV infection and disease after allo-HSCT relies on the interaction of several factors pertinent to the virus, the host, and the type of transplant. 6,7 Among virus-related factors, the development of CMV disease has directly been correlated with viral replication and related CMV DNAemia. Considering the antiviral strategies to prevent CMV disease, prophylaxis and pre-emptive therapy are commonly recognized as current standard approaches.…”
