Background: Acute graft-versus-host disease (aGVHD) is the most frequent and severe complication after allogeneic hematopoietic stem cell transplantation (HSCT). Objective: To determine if a history of prior infection in the allogeneic HSTC recipient is a risk factor to develop aGVHD. Methods: A retrospective cohort study based on data collected from the Department of Hematopoietic Stem Cell Transplantation at the Instituto Nacional de Pediatría (INP) from January 1998 to December 2016 was performed to identify if prior infection was a predictive risk factor for aGVHD. Results: 27 patients developed aGVHD (36.4%). Median time to aGVHD presentation was 82 days (9 to 273 days). Most patients developed grade III aGVHD. Following the multivariate analysis peripheral blood > bone marrow (OR 12.3; p < 0.001), cell dose > 8.3 × 10 6 /kg (OR 7.1; p = 0.05), peripheral blood (OR: 11.4; p < 0.001), infection 3 months prior to allogeneic transplant (OR: 4.5; p < 0.03) and CMV infection in the recipient (OR: 4.68; p < 0.03) were significant. Conclusions: Either bacterial infection or CMV infection in the recipient was significant risk factor within the aGVHD recipients; it is important to consider these factors for patients that are going to receive an allogeneic HSCT.

Background: Hematopoietic stem cell transplantation (HSCT) is an effective method for the treatment of hematological malignancies, severe aplastic anemia, and myelodysplastic syndromes. The most common infectious complication after HSCT is cytomegalovirus (CMV) infection. The purpose of this study was to analyze the status of research related to CMV infection after HSCT by conducting a literature search for CMV, hematopoietic, and stem cell transplantation. Methods:The Science Citation Index Expanded (SCI-E) database in the Web of Science Core Collection (WOSCC) was used as the target database for our literature search. The subject search terms were CMV, hematopoietic, and stem cell transplantation, with the logical operation 'AND'. The search date range was from 1900 to June 15, 2021. We used CiteSpace software to analyze the literature. The analysis included: the annual change in the number of publications, the annual change in the number of references cited, the distribution of countries, the distribution of institutions, the distribution of journals, the distribution of authors, and the use of keywords.Results: A total of 1,476 relevant documents were retrieved. Human Cytomegalovirus (CMV) remains a major cause of morbidity and mortality after allogeneic hematopoietic stem cell transplantation (allo-HSCT). Frequency of CMV infection and disease after allo-HSCT depend on several factors including virus-host interactions, recipient-donor CMV serology, donor type (HLA-identical vs. haplo-identical donors)and intensity of conditioning regimens. Moreover, monitoring strategies, cutoff values of viremia to guide prophylactic, preemptive, and treatment interventions remain to be determined. Broader knowledge in CMV biology and its relationship with the host immune-system is greatly contributing to develop novel therapeutic approaches including the application of novel antiviral drugs, with improved toxicity profiles currently under investigation in randomized phase II and III trials, and cell therapy approaches. Significant evidence of the clinical efficacy of adoptive T-cell therapy (ATCT) from CMV-seropositive donors is also emerging. Alternative methods allow ATCT also in allo-HSCT recipients of seronegative donors and cord blood grafts. Vaccine-immunotherapy has the difficult task to reduce the incidence of CMV reactivation/infection in immunocompromised allo-HSCT patients and reach CMV immune control. In this manuscript, we update a previous review by also including recent developments in the virology lab and their possible association with treatment strategies at bedside.
