Factors affecting visual outcomes in patients with diabetic macular edema treated with ranibizumab

Abstract: and the READ-2 Study Group 2 AbstractPurpose To identify factors associated with visual outcomes in patients with diabetic macular edema (DME) treated with ranibizumab (RBZ) in the Ranibizumab for Edema of the mAcula in Diabetes-Protocol 2 (READ-2) Study. Patients and methods Optical coherence tomography scans, fundus photographs, and fluorescein angiograms (FAs) were graded and along with baseline characteristics were correlated with month

“…[20][21][22] An analysis of fluorescein angiograms and other factors showed that patients who had poor visual acuity at the 2-year end point either had unresolved edema or were likely to have received laser burns within 1 mm of the fovea. 23 In this study, the CG had a decrease in BCVA with an overall decrease in CMT of 45.5 mm, which was not significant with pairwise comparison. Nineteen of 30 patients who received focal grid laser had a mean loss of 5.8 ± 10.2 letters compared with a gain of 0.9 ± 13.4 letters in the 11 patients who did not receive any focal laser.…”

“…In cases of chronic macular edema the effectiveness of treatments is reduced resulting in persistent edema and vision loss. Up to a quarter of patients may have a poor response to anti-VEGF, mainly related to chronicity of macular edema and a lower visual acuity before treatment is started (Channa et al, 2014). In such cases the local administration of steroids may have a greater affect and should be considered the treatment of choice.…”

“…11 Third, delaying effective treatment increases the risk of permanent vision loss, because chronicity of DME is associated with foveal atrophy and photoreceptor damage. 31 Members of the panel agreed that the balance of VEGF versus other inflammatory cytokines appears to be an important factor contributing to the different levels of anti-VEGF response observed in patients with DME. The importance of this balance goes hand-in-hand with the importance of the chronicity of DME in the variability observed in patient response to anti-VEGF treatment, because it is generally believed that DME is primarily mediated by VEGF in its early phase, whereas the involvement of other cytokines and inflammatory mechanisms becomes more important in chronic disease.…”
