IntroductionAs a consequence of the WHO Ottawa Charter of 1986, Industrial Safety and Health Law was revised in Japan in 1988. It prescribes that execution of health promotion measures for workers is an obligation of employers. In line with this, Total Health Promotion Plan (THP) has been instituted 1) . THP includes not only medical examination, but also investigation of life-style, and assessment of physical fitness. Determination of the maximal oxygen uptake (V · O 2 max) as an indication of a whole body endurance is included in physical fitness assessment. Based on these results, guidance for health care and psychological counseling are provided to employees. It is reported that V · O 2 max is influenced by life-style, such as exercise and smoking 2-9, 30, 32, 33) . Moreover, the preceding researches reported that the fall in V · O 2 max is closely connected with cardiovascular system disorders, such as heart diseases, and hypertension 7,[10][11][12][13][14][15][31][32] . Based on this evidence and from a viewpoint of preventive medicine, V · O 2 max is being used as a health index in THP.

Work Form Affects Maximum Oxygen Uptake for One Year in Workers

Our previous study suggested that the work form is related to the maximum oxygen uptake (VO 2 max). Therefore, we test the hypothesis by a longitudinal study that, besides these lifestyle-related factors, the work form increases VO 2 max among workers. Seven hundred and ninety nine males aged 37.1 ± 10.6 and 395 females aged 37.7 ± 10.5 (range: 20 to 59 yr old), who participated in The Total Health Promotion Plan (THP) at their respective workplaces twice in one year from 1997 to 1998, were selected. Data on VO 2 max were analyzed for age, systolic blood pressure (SBP), body mass index (BMI), work form (sedentary, standing, and ambulatory), exercise, and smoking. Age and the baseline variables for BMI and for VO 2 max showed significant negative relationships with a change in V O 2 max in males and females. Further, a change in the BMI showed a significant negative relationship with a change of VO 2 max in males. However, exercise habit and an ambulatory work form showed significant positive relationships with a change of V O 2 max in males. It was suggested that after adjusting for other factors in males, an ambulatory work form may be associated with an increase in VO 2 max in one year.

Background Poor cardiorespiratory fitness is a risk factor for cardiovascular morbidity. Alcohol consumption contributes substantially to the burden of disease, but its association with cardiorespiratory fitness is not well described. We examined associations between average alcohol consumption, heavy episodic drinking and cardiorespiratory fitness. Design The design of this study was as a cross-sectional population-based random sample. Methods We analysed data from five independent population-based studies (Study of Health in Pomerania (2008-2012); German Health Interview and Examination Survey (2008-2011); US National Health and Nutrition Examination Survey (NHANES) 1999-2000; NHANES 2001-2002; NHANES 2003-2004) including 7358 men and women aged 20-85 years, free of lung disease or asthma. Cardiorespiratory fitness, quantified by peak oxygen uptake, was assessed using exercise testing. Information regarding average alcohol consumption (ethanol in grams per day (g/d)) and heavy episodic drinking (5+ or 6+ drinks/occasion) was obtained from self-reports. Fractional polynomial regression models were used to determine the best-fitting dose-response relationship. Results Average alcohol consumption displayed an inverted U-type relation with peak oxygen uptake ( p-value<0.0001), after adjustment for age, sex, education, smoking and physical activity. Compared to individuals consuming 10 g/d (moderate consumption), current abstainers and individuals consuming 50 and 60 g/d had significantly lower peak oxygen uptake values (ml/kg/min) (β coefficients = -1.90, β = -0.06, β = -0.31, respectively). Heavy episodic drinking was not associated with peak oxygen uptake. Conclusions Across multiple adult population-based samples, moderate drinkers displayed better fitness than current abstainers and individuals with higher average alcohol consumption.
