BackgroundVitiligo is an acquired depigmented skin disease resulting in white macules, which may significantly impair the quality of life (QoL) of the patients.ObjectiveTo estimate the QoL in Chinese vitiligo patients using camouflage with a more detailed description, and to identify the possible risk factors related to poor QoL.MethodsAn online survey was conducted in vitiligo patients using camouflage from a vitiligo community. Survey questions included demographic, clinical information, dermatology- and vitiligo-specific QoL questionnaires. Multivariate logistic analysis was performed to identify risk factors that related to poor QoL.ResultsIn total, 884 respondents were included in the analyses, of which 413 (46.7%) were male. The score of DLQI was 5.83±5.75 (mean± SD). Age, gender, marriage status, occupational status, anogenital involvement, patient-perceived severity (presented by VAS score), symptoms as itching, pain, sunburn and koebner phenomenon, total cost of treatment and degree of satisfaction in camouflage therapy were independently associated with DLQI score (p<0.05).ConclusionVitiligo has considerable impact on QoL of affected patients in Chinese population even when they were using camouflage. Camouflage might be helpful to improve QoL of the patients.

Since localized treatment for vitiligo is as essential as systemic treatment, a reliable instrument for target evaluation is needed besides those for whole body evaluation. We developed the Vitiligo Extent Score for a Target Area (VESTA) using reference images of both marginal and perifollicular repigmentation to measure the repigmentation rate (%) in a target lesion. In the validation study, a total of 65 dermatologists in 10 institutes evaluated 17 pairs of vitiligo images (pre- and post-treatment) using both a rough estimate and the VESTA. The VESTA (concordance correlation coefficient: 0.949, 95% confidence interval [CI] 0.942-0.955) was significantly more accurate than the rough estimate (0.896, 95% CI: 0.883-0.908). It was also associated with better inter-rater reliability over the rough estimate, albeit not significant. The VESTA can afford intuitive, convenient, and reliable assessment of the treatment response in a target area, and would be useful in clinical practice as well as retrospective studies.

A Report on Applying Digital Epiluminescence Dermoscopy to Guide Topical Treatment for Generalised Vitiligo

We have reported on utilising digital epiluminescence dermoscopy to detect the sparing of perifollicular pigments in localised vitiligo. These spotty zones might be the most acute autoimmune battle line. We saw a 68-year-old gentleman with five-year history of segmental/generalised vitiligo. He declined systemic treatments. These spotty zones mentioned above were identified by epiluminescence dermoscopy. We advised him to apply a very potent topical corticosteroid two times per week to these zones. Much improvement was seen in two months. Complete remission was achieved in another two months. We discussed on the roles of epiluminescence dermoscopy on the diagnosis and treatments of patients with all types of vitiligo where the perifollicular pigment sparing zones were too small to be identifiable by naked eyes.
