Create an overview of characteristics of patients in long-term forensic psychiatric care (LFPC) with a higher length of stay (LOS) care compared to patients in regular forensic psychiatric care (RFPC) with a shorter LOS. Data were collected from 139 patient records. This study examined whether patients in LFPC differ from patients in RFPC on sociodemographic data, legal data and clinical data and whether those characteristics are able to predict LOS. Patients in LFPC were more often born in a Dutch Caribbean country, less often had a substance abuse disorder, were more often emotionally neglected during childhood, had a higher HCR-20 risk item score, a higher security needs score, a higher (less successful) recovery score, were more often recidivist and had absconded more often than RFPC patients. Certain characteristics were able to distinguish the longer LOS group which might be useful to establish sequel services and enhance treatment efficiency.

It is the responsibility of forensic psychiatric hospitals to detain and treat patients, gradually reintegrating them into society; decisions to release patients must balance risk to the public with maintaining the least restrictive environment for patients. Little is known about the factors considered when making such decisions and whether these factors have been empirically linked to future risk of violence. The current study explores the factors predictive of forensic treatment teams' recommendations for patients under the care of the Ontario Review Board (ORB). Factors differ depending on level of security; decisions on medium secure units were influenced by the presence of active symptoms and patients' overall violence risk level and decisions made on minimum secure units were influenced by the number of critical incidents that occurred within the recommendation year. Understanding the factors used to make recommendations to the ORB tribunal helps treatment teams to reflect on their own decision-making practices. Furthermore, the results serve to inform us about factors that influence length of stay for forensic psychiatric patients. Copyright © 2016 John Wiley & Sons, Ltd.

Op 10 juni vond in VU medisch centrum de eerste patie¨n-tendag van de IC-afdeling plaats. Voor deze dag waren patie¨nten uitgenodigd die in de periode vanaf 2003 zeven dagen of langer opgenomen waren geweest op de IC, samen met hun partners, kinderen of andere relaties.VU medisch centrum had hiermee een primeur: niet eerder werd in Nederland een dag georganiseerd specifiek gericht op deze doelgroep. Er waren ruim 170 bezoekers aanwezig. De patie¨ntendag had als titel 'De IC door de ogen en oren van de patie¨nt' en ging vooral in op de psychologische aspecten die een IC-opname voor de patie¨nt en zijn/haar naasten tijdens de opname, maar ook daarna, met zich meebrengt. Zo vertelden een ex-patie¨nt en zijn vrouw over hun ervaringen, kwam een psycholoog aan het woord over het ontstaan en de functie van dromen en ging een psychiater in op het ontstaan van onrust, verwardheid en delier. Daarnaast werd een film getoond, gemaakt door een IC-verpleegkundige, die de IC vastlegde vanuit het gezichtspunt van een patie¨nt. Verder konden de bezoekers een fototentoonstelling bekijken over een dag op de IC en was er gelegenheid voor een ontmoeting met de medewerkers van de IC. Tijdens de dag kwam duidelijk naar voren dat patie¨nten e´n hun naasten behoefte hebben aan individuele nazorg, specifiek gericht op de IC-periode. De patie¨nt heeft zelf vaak geen bewuste herinneringen aan deze periode, maar heeft achteraf dikwijls last van onrust en dromen. De partner wordt tijdens de opname e´n daarna geconfronteerd met de verwardheid en het veranderende gedrag van de patie¨nt en kan dit niet delen omdat hij/zij zelf dit niet bewust heeft meegemaakt. Binnen VU medisch centrum zijn plannen voor het starten van nazorgtrajecten voor IC-patie¨nten. Deze patie¨ntendag heeft duidelijk gemaakt dat daar zeker behoefte aan is.Critical care (2007) 04:137
