BackgroundNon-adherence to tuberculosis (TB) treatment is an important barrier for TB prevention and control. Poor adherence may result in prolonged disease infectiousness, drug resistance, relapse and death. The aim of this study was to assess factors influencing adherence to tuberculosis treatment in selected health facilities in Asmara, Eritrea.MethodsA qualitative study which included in-depth interviews with 12 TB patients, three focus group discussions in selected health facilities in which one group comprised eight patients and key informant interviews with three health workers. Data analysis was done by translating and transcribing the verbatim of the interviews and focus group discussions. Transcribed data was then analysed using thematic framework procedure.ResultsThis study found that patients lacked knowledge about the cause, transmission and duration of treatment of TB. The most common reason mentioned for discontinuing treatment was the patient “felt cured”. Almost half of the respondents did not know the standard treatment duration and the consequences they face if they halt treatment. Patients reported losing their job when their diagnosis was known, were too ill to continue working or unable to find daily work due to time-consuming treatment arrangements. With few exceptions, the majority of patients reported that the short distance to the clinic encouraged them to attend regular treatment follow-up. Most of the respondents were unable to get enough food, leading to stress and feelings of hopelessness. Lack of social support for most of the patients was a critical factor for adherence as were stigma, medication side effects and long treatment duration. Recognized as an enabler to treatment adherence, health workers had good communication and positive attitude towards their patients.ConclusionLack of knowledge, loss of income, stigma and lack of social support, drug side effects and long treatment duration emerged as important barriers for treatment adherence. Short distances to health facilities, good communication and accepting attitude of health care providers emerged as enablers for treatment adherence. For better treatment adherence, comprehensive health education at treatment sites, patient’s family members and the community at large and strengthening of social support structures need to be addressed.

To describe the follow-up treatment of Resistant Rifampicin in health care facilities based on the results of the gene-Xpert examination. The study design was cross-sectional using data from "Evaluation study of detection of TB cases with the rapid molecular test in Indonesia in 2018". The data resources were from 42 hospitals and two primary health care facilities having conducted the gene-Xpert examination at least six months in 42 districts, 26 provinces in Indonesia. The number of TB diagnoses with gene-Xpert from health facilities was 33,630 cases, 31.6% of those cases were TB positive, namely rifampicin sensitivity of 89% (9,456) and rifampicin-resistant of 11% (1,171). 29% of resistant rifampicin tuberculosis cases were missing cases consisting of the untreated and unknown follow-up treatment of 18% and 11%, respectively. Missing cases were mostly found in males of 66.1%. Based on the age, 76.9% of the missing cases were in productive age, such as 15 to 54 years. Moreover, according to the TB treatment history and origin of the case, missing cases were found in new cases, 54.4%, and the existing health facility 51.2%. The most reason for missing the RR-TB case in the follow-up treatment was the refusal of treatment 40.3%. The high non-compliance to TB treatment was caused by inadequate general knowledge about TB, lack of social support, medication side effects, and long treatment period, which were posed as barriers to adherence to treatment.

Introduction. One third of the increase in tuberculosis cases is attributed to the spread of HIV. Objective. To describe the Tb/HIV coinfection in the department of Meta from 2010 to 2015. Materials and methods. We conducted an observational, descriptive and retrospective study. After selecting 219 cases for analysis, two new databases were constructed and analyzed in three phases: Identification of sociodemographic and clinical characteristics, indicators by municipality (prevalence and therapeutic success) and stratification in epidemiological scenarios according to the prevalence (burden) of the illness. Results. Sixty percent of the municipalities corresponded to scenario 2. People with Tb/HIV coinfection who had not been treated previously, had 2.39 times more probability of having therapeutic success compared to those previously treated, this association being statistically significant (RP=2,39; 95% CI 1,3-9,6; p=0,01). Conclusion. Stratification by epidemiological scenarios is useful for planning prevention and control activities.
