Faciobrachial dystonic seizures: the influence of immunotherapy on seizure control and prevention of cognitive impairment in a broadening phenotype

Abstract: Voltage-gated potassium channel complex antibodies, particularly those directed against leucine-rich glioma inactivated 1, are associated with a common form of limbic encephalitis that presents with cognitive impairment and seizures. Faciobrachial dystonic seizures have recently been reported as immunotherapy-responsive, brief, frequent events that often predate the cognitive impairment associated with this limbic encephalitis. However, these observations were made from a retrospective study without serial cog… Show more

“…Two patients had antibodies targeting CASPR2. It is well known that FBDS were reported to precede LGI1 antibody‐associated encephalitis, suggesting that early immunotherapy could prevent the evolution to limbic encephalitis 12, 25. We identified similar FBDS in 6 (28.57%) of 21 LGI1‐seropositive patients in this cohort, often accompanied by other seizure semiologies including SPS and CPS.…”

“…Patients with LGI1 antibodies often have a novel seizure type, faciobrachial dystonic seizures, during or even preceding the limbic encephalitis [52,53]. Prompt recognition of these seizures and immunomodulatory treatment may prevent progression to limbic encephalitis [53].…”

“…While patients with early immunotherapy may have a good prognosis [2,3], cognitive deficit develop in most [4,5]. Structural magnetic resonance imaging (MRI) showed extratemporal T2 hyperintensities in the early phase with faciobrachial (dys)tonic seizures (FBDS) and reduced volume of the whole brain one year after FBDS [6][7][8][9][10]. Glucose metabolism is also abnormal in areas outside of the limbic structures [8,[11][12][13].…”
