Extremity Soft Tissue Sarcoma in a Series of Patients Treated at a Single Institution

Abstract: Quality of surgical margins independently predicted local control and survival. The effect on survival was directly mediated by local recurrence to proximal sites invading the abdomen/thorax, and this may indeed be the main way by which quality of surgery directly impacts the final prognosis of ESTS patients.

“…This was equal to or lower compared with previous studies (22%-49%) [4,22,25,36,37,43,51,55,56,58,65]. Margin status did not have a statistically negative effect on the rate of metastases; however, given the small number of R1 margins, a conclusion cannot be drawn from this either.…”

“…There were five local recurrences (9%) among our patients, which was lower than numerous studies that evaluated local recurrences in patients with soft tissue sarcomas (13%-39%) [4,22,25,36,37,43,51,55,56,58,65]. Some of these studies included all tumor grades, superficial and deep tumors as well as trunk wall and retroperitoneal tumors.…”

“…Investigators have attempted to assign quantitative values to various qualitative tissue planes in an effort to determine the appropriate clinical resection guidelines [10][11][12]. Few authors have looked specifically at the relationship between close but negative margins and local recurrence [2,8,14,16,18,19,21]. Evaluation of the literature is complicated by varying inclusion and exclusion criteria and numerous confounders.…”

“…Patients who develop a local recurrence from a soft-tissue sarcoma have a poor prognosis [1,7,8]. One study suggests the development of a local recurrence is the most important factor associated with decreased survival, and once patients develop a local recurrence, they are about three times more likely to die from disease compared to patients who do not develop a local recurrence [6].…”
