Extrathoracic Mesothelial Proliferations and Their Mimics

Abstract: Mesothelial proliferations, either reactive or neoplastic in nature, often pose difficult diagnostic dilemmas. Electron microscopy continues to be a gold standard in the identification of mesothelial differentiation. However, it is very common to apply long panels of antibodies for that purpose. In most cases, light microscopy and immunohistochemistry will solve the problem. However, the definitive, specific, and sensitive immunohistochemical marker is still lacking. This is particularly true in peritoneal and… Show more

“…However, the wide morphologic diversity is a well-recognized feature [25]. The most commonly cited differential diagnosis include carcinoma of the rete testis, ovarian epithelial-type tumours of testis and paratestis, secondary adenocarcinomas, adenomatoid tumour and pleomorphic sarcomas [22,24,26,27]. In our case, the pre-operative biopsy specimen lacked common features of mesothelioma, while the sex cord-like appearance (i.e.…”

“…Cellular nuclear pleomorphism and papillary configuration are signs of lethal potential [52]. An infiltrative pattern of growth with increased cellularity nuclear pleomorphism and high mitotic rate and stromal invasion [105, 195]. Large lymphoid cells with clear or slightly eosinophilic cytoplasm with large strongly atypical polymorphic nuclei and a great number of mitoses; epithelioid features with papillary growth, papillary tubules, and solid growth in invasive foci [196].…”

“…Moreover, even though electron microscopy (transmission electron microscopy—TEM) is an ancillary technique mainly used in the field of scientific research [ 18 ], it may continue to have a purpose in the pathological diagnosis of difficult cases, as also reported in the latest statement for the pathological diagnosis of mesothelioma [ 19 ]. In fact, with ultrastructural examination, it is possible to identify cellular details that reveal the mesothelial origin, such as elongated and branching microvilli as well as giant desmosomes and evident intracytoplasmic tonofilaments [ 20 , 21 , 22 , 23 , 24 ]. Scanning electron microscopy (SEM) equipped with an X-ray microanalysis system (EDS) on pathological specimens may also be considered as an additional tool for the research of fibers or crystals eventually attributable to a previous exposure history.…”
