Extraspinal Bone and Soft-Tissue Tumors as a Cause of Sciatica

Abstract: A high index of clinical suspicion is the key to early diagnosis of bone or soft-tissue tumors as a cause of sciatica; special attention should be given to pain pattern, physical examination of the entire course of the sciatic nerve, and selection of proper imaging studies. Routine anteroposterior plain radiography of the pelvis as part of the initial imaging screening process is recommended.

“…The nerve divides into the tibial and common peroneal nerves in the lower third of the posterior thigh to give motor supply to the knee flexors and all the muscles of the lower leg. It also supplies cutaneous innervation to the posterior thigh, lower leg, and foot [23,24].…”

“…Common etiologies of sciatica can further be grouped into intraspinal (herniated nucleus pulposus, central canal stenosis, degenerative hypertrophy of the facet joints, degenerative synovial cysts of the spinal canal, epidural abscesses, and primary and metastatic neoplasms) and extraspinal sources (lumbosacral involvement by retroperitoneal tumors, endometriosis, and viral infections) [27]. Unusual extraspinal etiologies of sciatica include nerve sheath tumors, sciatic hernias, intrapelvic aneurysms, and large uterine leiomyomas [24,25,28].…”

“…1 However, some intraspinal or extraspinal pathologic processes along the sciatic nerve may also cause sciatica. Whereas lumbar spine imaging reveals the causes of intraspinal non-discogenic sciatica (NDS), extraspinal sciatica is often misdiagnosed because routine diagnostic tests focus on the lumbar spine.…”
