Extrapelvic bowel resection and anastomosis in cytoreductive surgery for ovarian cancer

Abstract: Ovarian cancer is known as the second most common cause of death among gynecologic cancers.Survival outcome has been reported to be significantly associated with the efforts to minimize residual disease after cytoreductive surgery. As ovarian cancer often invades the small and large bowel without boundary, bowel surgery has been a crucial part of the cytoreductive surgery to achieve complete tumor removal. The scope of surgical resections has progressively expanded to include small and large bowel resections, … Show more

“…For skilled gynecologic oncologists, the scores of diffuse small bowel adhesions/thickening and surgical difficulty were not completely consistent. Bowel resection surgery can be performed better by skilled gynecologic oncologists (15). Removing retroperitoneal lymph nodes above the IMA that are not adhered to blood vessels also cannot stuff skilled gynecologic oncologists.…”

“…Ovarian cancer is prone to involve the small intestine or colon, so resection of intestinal metastases is a key part of cytoreductive, and intestinal surgery includes intestinal resection, intestinal tumor stripping. The decision of whether to perform intestinal resection and the extent of resection often needs to be made in conjunction with the intraoperative situation [39]. Reports of intestinal resection have been published to increase optimal debulking surgery rates and improve patient survival [40] [41], however, extension of the surgical scope is associated with higher rates of intraoperative and early postoperative complications (e.g., bleeding, anastomotic dehiscence, and infection) [42].…”

“…Bowel metastasis frequently occurs with advanced ovarian cancer, and debulking cytoreduction surgery, especially en bloc cytoreduction, is recommended ( 1 – 3 ). Bowel resection is performed when bowel metastasis is observed before or during surgery, and it is followed by primary anastomosis or ostomy ( 4 7 ). Primary anastomosis is the first choice after bowel resection, and ostomy is the alternative choice.…”
