Abstract: Extraosseous or extraskeletal Ewing’s sarcoma is a very rare mesenchymal soft tissue malignancy. We report on a case of abdominal swelling in an 80-year-old woman caused by a large intra-abdominal EES. This case report illustrates that Ewing’s sarcoma can occur in adults and elderly, and highlights the importance of early diagnosis because of poor prognosis.

Introduction: Ischial lesion is considered rare. Aim: In this study, we review the patient demographics, imaging appearances, and pathological entities which arise in the ischium with the aim of facilitating a differential diagnosis. This allows appropriate management to be instigated in a timely fashion, enabling lesion to be stratified into those that can be managed locally or referred to a specialist center. Materials and Methods: A retrospective search of our oncology and radiology database was performed to identify primary ischial lesions. Results: The search revealed 82 cases with primary involvement of ischium. The most common benign tumor was aneurysmal bone cyst in patients under 40 years and osteochondroma in patients over the age of 40 years. Metastasis was the most common malignant tumor in both cohorts (<40 years and over 40 years). Conclusion: We present the patient demographics, imaging appearances of pathological entities which arise in the ischium with the aim of facilitating a differential diagnosis.

Extraskeletal Ewing's sarcoma (EES) is a rare soft tissue tumor predominantly observed in adolescents and young adults, and is characterized by aggressive behavior. So far, only two cases of primary axillary soft tissue EES have been reported in the literature. One of them was a 29-year-old female patient who presented with a lump in her left axilla. Upon examination, an irregular, painless mass, measuring 5 cm × 5 cm × 3 cm, was noted in the left axilla. A histopathological examination of the mass revealed small, round, blue cells with scant cytoplasm, round nuclei, numerous mitosis, and necrosis. An immunohistochemistry (IHC) examination was positive for CD99 and negative for ER, PR, Her2neu, CK7, CK5/6, CD56, CD45, CK-pan, CKHMW, P63, desmin, S100, TdT, vimentin, myogenin, synaptophysin, and chromogranin A. The patient was diagnosed with primary axillary soft tissue EES and was started on neoadjuvant chemotherapy. Twelve months later, she is clinically free from the disease.
