Introducción. El linfoma primario del cuello uterino es una patología infrecuente, de síntomas inespecíficos y que pocas veces altera el examen de citología del cuello uterino dado que se desarrolla en el estroma cervical. Para su tratamiento existen varias opciones, incluyendo quimioterapia, radioterapia y cirugía, así como combinaciones de estas. La localización única del linfoma en el cuello uterino se considera un factor de buen pronóstico. Presentación del caso. Paciente femenina de 49 años, quien consultó por dolor pélvico y flujo y sangrado genital. Se ordenó colposcopia por reporte de ASC-H (células escamosas atípicas que no excluyen lesiones intraepiteliales de alto grado) en citología vaginal. La biopsia reportó linfoma difuso de células B grandes, el cual se trató con rituximab, ciclofosfamida, doxorrubicina, vincristina y prednisolona por tres ciclos, y con rituximab, ifosfamida, carboplatino y etopósido por dos ciclos; este cambio se hizo debido a una mala respuesta con el primer esquema. Se realizó nueva biopsia después del último ciclo de quimioterapia con reporte de pólipo de tipo endocervical y abundantes grupos de células glandulares con atipia focal. Los estudios de imágenes diagnósticas posteriores al tratamiento reportaron engrosamiento concéntrico de la unión entre cuello uterino y vagina. A los 7 años del diagnóstico del linfoma se realizó otra biopsia que resultó negativa para displasia o malignidad. Al momento de la elaboración del presente reporte, 10 años después del diagnóstico, la paciente se encontraba asintomática y libre de enfermedad. Conclusiones. El linfoma primario del cuello uterino es una patología rara que en pocas oportunidades se evidencia con anormalidad en la citología vaginal como en el caso reseñado. Dado este hallazgo se realizó una colposcopia mediante la cual se confirmó el diagnostico de linfoma difuso de células B grandes. Se presenta un caso con evolución satisfactoria y supervivencia libre de enfermedad después de 10 años.

Prognostic Significance of Clinicopathological and Immunophenotypic Properties of Diffuse Large B Cell Lymphoma Patients

Objective: Diffuse large B-cell lymphoma (DLBCL) accounts for approximately 30% of non-Hodgkin lymphoma (NHL) in adults. In our study, the clinicopathological features of patients with DLBCL were evaluated retrospectively. Methods: The study included 43 patients with the diagnosis of DLBCL between 2009-2016. Clinical and histopathological features of the patients were collected from the archive of our hospital. Results: The mean age of the patients was 62.5 (23-86) years. Twenty-four of them (55.8%) were male and 19 (44.2%) were female. Twenty-five patients (58.1%) had B symptoms. Early stage (I and II) disease was seen in 21 (48.8%) patients, while advanced stage (III and 1V) disease was seen in 22 (51.2%) patients. Bone marrow involvement was present in 5 (11.6%) patients. Nodal involvement was present in 39.5% and extranodal involvement in 60.5% of the patients. The most commen involved node was cervical lymph node and the most involved extranodal site was gastrointestinal system and especially stomach. Immunohistochemically, BCL2 and BCL6, which had prognostic significance, were expressed as 46.5% (n=20) and 27.9% (n=12) of the cases, respectively. A significant relationship was found between Bcl6 protein expression and gender (p<0.001, almost all female patients) and extranodal involvement (p=0.009). The patients were in a low risk group with a rate of 58.1% and a high risk of 41.9% according to the İnternational Prognostic Index (IPI). Twenty-seven out of 43 patients exited. The mean survival rate was 37.2 percent. Conclusion: In this study, there was no correlation between the histopathological features and mean life span, but an inverse relationship was found with IPI score. In our study, clinical and immunophenotypic features and significant prognostic parameters of DBBHLs were emphasized.

Diffuse large B-cell lymphoma of the vagina in pregnancy

A 28-year-old primigravida was evaluated for complaints of difficulty urinating and pelvic pain of 6-weeks duration. She denied fever, night sweats, weight loss or fatigue. Pelvic ultrasonography revealed a single fetal pole with cardiac activity and a 7 cm mass in the anterior vagina which encased the urethra. The diagnosis of diffuse large B-cell lymphoma germinal centre type was made on analysis of biopsied pelvic mass. Whole body MRI revealed the disease was limited to the vagina. The patient received six cycles of Rituximab-cyclophosphamide, doxorubicin, vincristine and prednisone with significant improvement in her symptoms. Serial ultrasounds over the subsequent months showed appropriate development of the fetus. Whole body MRI after treatment showed decreased size and decreased signal of the primary pelvic mass compatible with favourable treatment response. Challenges in the management of this rare presentation of lymphoma are discussed.
