StandorteDer extramammäre Morbus Paget (EMPD) ist eine seltene Erkrankung. Er tritt bevorzugt bei Frauen sowie bei der weißen Bevölkerung im Allgemeinen auf, mit einem Erkrankungsgipfel bei den 50-bis 80-Jäh-rigen [1]. Es wurde nachgewiesen, dass der EMPD im Hinblick auf die histologischen und klinischen Faktoren und das Potenzial zur Ausbildung von Metastasen als heterogene Erkrankung betrachtet werden muss. Er ist zudem häufig mit synchronen oder metachronen invasiven Karzinomen assoziiert. > Der extramammäre Morbus Paget, eine seltene Erkrankung, betrifft bevorzugt Frauen und die weiße Bevölkerung Der EMPD tritt in apokrinen Drüsen auf. Die am häufigsten befallenen Lokalisationen sind Vulva, Perineum, Perianalregion, Skrotum, Penis oder Schambereich [1, 2]. Es fand sich auch ein familiärer EMPD (7 Fälle), obwohl die genetischen Grundlagen noch unbekannt sind [3]. Der Morbus Paget wird wegen seiner Assoziation mit verschiedenen synchronen oder metachronen Malignomen als komplexe Störung betrachtet, bedingt durch die bezüglich klinischer und histologischer Aspekte unterschiedlichen pathologischen Prozesse. Die Basis dieser Studie bilden 6 Fallberichte sowie eine Durchsicht der Literatur von 1990 bis 2008, um neue Erkenntnisse über diese Erkrankung zu gewinnen. Eigene Beobachtungen Von März 1996 bis Dezember 2006 wurden 6 Patienten mit perianalem Morbus Paget behandelt. Fünf dieser Patienten waren schon Gegenstand einer neueren Publikation [4]. Es handelte sich um 3 Männer und 3 Frauen. Das Durchschnittsalter betrug 77 Jahre. Fünf dieser Patienten hatten einen EMPD. In einem Fall lag ein distales rektales Adenokarzinom in Verbindung mit einer pagetoiden Ausbreitung vor. Die vor allem berichteten Symptome waren Schmerzen und perianales Brennen. In einem Fall war die Erkrankung mit Blutungen aufgrund eines Rektumkarzinoms assoziiert. Die präoperativen Maßnah-men umfassten die Messung des karzinoembryonalen Antigens (CEA), eine Koloskopie sowie eine Computertomographie des Abdomens, um das Vorliegen von Metastasen oder assoziierten Karzinomen auszuschließen. Dieselbe Analyse wurde während der Nachbeobachtung durchgeführt, um die anschließende Entwicklung anderer Tumoren, Lymphknoten oder einer Metastasierung auszuschließen. Fall 1Bei einer 78-jährigen Frau mit einer ausgedehnten und zirkulären perianalen Lä-sion lag ein für Morbus Bowen positiver Biopsiebefund vor. In einer anderen Abteilung unterzog sich diese Patientin einer ausgedehnten Lokalexzision und Rekonstruktion des Defekts mit einem Hauttransplantat. Die histologische und immunohistochemische Untersuchung schloss einen Morbus Bowen aus und ermöglichte die Diagnose eines EMPD mit beginnender Hautinvasion. Die durchgeführte Exzision ergab einen R2-Tumor, da eine Infiltration der Ränder rechts lateral und anterior vorlag. Zwei Jahre spä-ter entwickelte sich ein Rezidiv, das die perianale Haut und den Analkanal oberhalb der Linea dentata einbezog. The medical literature available to guide the management of empd is limited, particularly for metastatic disease. Another challenge is to differentiate metastatic empd from a concurrent invasive cancer. Here, we present our case series of 6 patients with empd of the penoscrotal region, our experience with systemic therapy, and a literature review for this rare malignancy. METHODSWe identified 6 cases of penoscrotal empd treated at The Ottawa Hospital from 2007 to 2012. All cases underwent pathology review. After research ethics board approval, clinical and demographic data, including age at presentation, presence of a second malignancy, management, and follow-up were collected from patient records. To report the surgical management, complications and prognosis of patients with penoscrotal extramammary Paget's disease (EMPD) at different clinical stages. Between 2003 and 2008, a total of 30 male patients with penoscrotal EMPD were enrolled and evaluated. All enrolled subjects received frozen biopsy-guided local wide resection and immediate reconstruction. Patients were followed every 3 months postoperatively. Among the 30 patients who accepted and underwent frozen biopsy-guided local wide resection treatment and reconstruction, two (6.7%) cases exhibited positive margins, verified by pathological examination, and underwent re-excision after surgery. The technique of primary closure or an adjacent flap was used in 10 (33.3%) cases, split-thickness skin grafts were used in 15 (50%), and an anterolateral thigh perforator flap was used in five cases (16.7%). The postoperative complications were acceptable. The mean follow-up time was 64.9629.6 months. Of all 30 cases, 22 patients (73.3%) survived with no evidence of recurrence, four patients (13.3%) exhibited local recurrence, two patients (6.7%) exhibited both local recurrence and distant metastasis and the remaining two patients (6.7%) exhibited distant metastasis. Five patients died from metastasis or cachexia. Current surgical techniques, including primary closure, adjacent flaps, split-thickness skin flaps and anterolateral thigh perforator flaps are able to reconstruct all types of defects with acceptable complications. Some patients with negative margins went on to exhibit local recurrence, potentially due to adnexal carcinoma or internal malignancy.
