Extralobar pulmonary sequestration presenting as hemothorax

Abstract: Symptomatic extralobar pulmonary sequestration (EPS) is extremely rare. Herein, we report two male patients (3 and 16 years of age) with EPS presenting as hemothorax. Thoracotomic resections of the sequestrated lungs were uneventful. Pathologic examinations revealed hemothorax caused by circulatory disorders within the EPSs.

“…It was first described in 1861 by Rokitansky and Rektorik. 2 There is no consensus regarding the etiology of PS, but there are two possible hypotheses: formation and caudal migration of a supernumerary pulmonary bud that accompanies the esophagus and primary pulmonary vascular deficiency, which would explain the persistence of systemic collateral circulation. 1,6 PS accounts for 0.15% to 6% of all congenital pulmonary malfunctions. These lesions are divided into two types on the basis of their pleural coverings.…”

“…Although PS is mostly asymptomatic in the neonatal period, severe respiratory distress syndrome, congestive heart failure, hydrothorax, pneumonia may happen in infancy. [1][2][3] In our case, the clinical findings were pulmonary hemorrhage and respiratory failure developed shortly after birth.…”

“…In one previous case, resection of the sequestrum required concomitant partial resection of the diaphragm due to dense adhesion 14 days after the clinical presentation [7]. Dense adhesion can complicate dissection from the surrounding organs, therefore early surgical resection is recommended.…”

“…1) Because the symptoms are not typical, pulmonary sequestration is often misdiagnosed as bronchiectasis, congenital emphysema, hemothorax, and even lung cancer. 2,3) Routine chest X-ray and computed tomography (CT) scan have difficultly showing abnormal blood vessels clearly. Threedimensional computed tomographic angiography (3D CTA) can depict an aberrant blood supply from systemic circulation to the consolidation accurately and intuitively, thereby greatly improving the diagnosis rate preoperatively and reducing the surgical risk.…”
