Meningiomas of the spinal axis have been identified from C1 to as distal as the sacrum. Their clinical presentation varies greatly based on their location. Meningiomas situated in the atlanto-axial region may present similarly to some meningiomas of the craniocervical junction, while some of the more distal spinal axis meningiomas are discovered as a result of chronic back pain. Surgical resection remains the mainstay of treatment, although advancements in radiosurgery have led to increased utilization as a primary or adjuvant therapy. Angiography also plays a critical role in surgical planning and may be utilized for preoperative embolization of hypervascular meningiomas.

Foramen magnum chordoid meningioma in a 22-year-old female

Chordoid meningioma is a rare variety of meningioma (0.5%–1%) with high rate of recurrence. They are grouped in WHO Grade II tumors. They are mainly distributed in the supratentorial location. There has been a single report of foramen magnum chordoid meningioma in a 3-year, 6-month-old child. Our patient, a 22-year-old female, admitted with progressive spastic quadriparesis with bowel and bladder involvement. She was operated with tumor resection done; postoperatively, patient showed dramatic improvement in the motor power as well as bowel and bladder function. Histopathology was suggestive of chordoid meningioma. She received 54 Gy of radiotherapy. She is doing well with no neurological deficits and no recurrence at 2-year follow-up.

Foramen magnum meningiomas are challenging tumors, requiring special considerations because of the vicinity of the medulla oblongata, the lower cranial nerves, and the vertebral artery. After detailing the relevant anatomy of the foramen magnum area, we will explain our classification system based on the compartment of development, the dural insertion, and the relation to the vertebral artery. The compartment of development is most of the time intradural and less frequently extradural or both intraextradural. Intradurally, foramen magnum meningiomas are classified posterior, lateral, and anterior if their insertion is, respectively, posterior to the dentate ligament, anterior to the dentate ligament, and anterior to the dentate ligament with extension over the midline. This classification system helps to define the best surgical approach and the lateral extent of drilling needed and anticipate the relation with the lower cranial nerves. In our department, three basic surgical approaches were used: the posterior midline, the postero-lateral, and the antero-lateral approaches. We will explain in detail our surgical technique. Finally, a review of the literature is provided to allow comparison with the treatment options advocated by other skull base surgeons.
