Extracorporeal Membrane Oxygenation in Infarct-Related Cardiogenic Shock

Abstract: Mortality in infarct-related cardiogenic shock (CS) remains high, reaching 40–50%. In refractory CS, active mechanical circulatory support devices including veno-arterial extracorporeal membrane oxygenation (VA-ECMO) are rapidly evolving. However, supporting evidence of VA-ECMO therapy in infarct-related CS is low. The current review aims to give an overview on the basics of VA-ECMO therapy, current evidence, ongoing trials, patient selection and potential complications.

“…Physiologically, systemic organ perfusion is improved. [3,4] Venoarterial extracorporeal membrane oxygenation therapy in acute myocardial infarction As a form of temporary MCS, VA-ECMO could provide cardiopulmonary support. In patients with AMI, VA-ECMO is suggested for temporary support with the potential for functional recovery.…”

“…3 Despite major improvements and advances in interventional and medical treatment of AMI, the mortality in infarct-related CS remains high, reaching 40-50%. 4,5 Anaemia is prevalent among cardiovascular patients and can lead to reduced oxygen tissue delivery, impaired oxygen utilization, and chronic tissue ischaemia. 6 patients worldwide and has been associated with adverse outcomes in cardiovascular diseases.…”

“…Microcirculatory hypoperfusion and reduced tissue oxygen delivery represent the most prominent steps in the pathophysiological cascade of CS and the degree of impairment is associated with unfavourable outcome in CS patients 3 . Despite major improvements and advances in interventional and medical treatment of AMI, the mortality in infarct‐related CS remains high, reaching 40–50% 4,5 …”
