Extracorporeal membrane oxygenation for severe ARDS in pregnant and postpartum women during the 2009 H1N1 pandemic

Abstract: The use of ECMO for severe ARDS in pregnant and postpartum women was associated with a 66% survival rate. The most common cause of death was bleeding. Infants delivered of mothers who had received ECMO had a 71% survival rate and, like their mothers, had no permanent sequelae at hospital discharge.

“…A c c e p t e d M a n u s c r i p t 13 Pandemic H1N1 underlined the potential interest of extra-corporeal membrane oxygenation in parturients [79]. Nevertheless, the risk of bleeding in the peripartum period makes this technique at risk [79,80].…”

“…There were no maternal deaths in their group. In another series reporting on the use of extracorporeal membrane oxygenation (ECMO) in cases of severe ARDS in pregnant or postpartum women, there was a 66% maternal survival rate [43].…”

“…However, a recent multicenter, randomized trial investigating the use ECMO for acute respiratory distress syndrome (ARDS) (CESAR) found that referral to centers with ECMO expertise was associated with significant improvement in survival in ARDS compared to previous studies (5). Several retrospective case series and studies since the CESAR trial have reported survival benefits of ECMO in patients with severe ARDS secondary to influenza (6)(7)(8)(9)(10)(11)(12)(13)(14).…”
