BackgroundCardiovascular magnetic resonance (CMR)-derived extracellular volume (ECV) and tissue tracking strain analyses are proposed as non-invasive methods for quantifying myocardial fibrosis and deformation. This study sought (1) to histologically validate myocardial ECV against the collagen volume fraction (CVF) measured from tissue samples of patients undergoing heart transplantation and (2) to detect the correlations between myocardial systolic strain and the myocardial ECV and histological CVF in patients undergoing heart transplantation.MethodsA total of 12 dilated cardiomyopathy (DCM) and 10 ischaemic cardiomyopathy (ICM) patients underwent T1 mapping with the Modified Look Locker Inversion recovery (MOLLI) sequence, T2 mapping and ECV. Myocardial systolic strain, including left ventricular global longitudinal (GLS), circumferential (GCS) and radial strain (GRS), were quantified using CMR cine images with tissue tracking analysis software. Tissue samples were collected from each of 16 segments of the explanted hearts and were stained with picrosirius red for histological CVF quantification.ResultsA strong relationship was observed between the global myocardial ECV and histological CVF in the DCM and ICM patients based on a per-patient analysis (r = 0.904 and r = 0.901, respectively, p < 0.001). In the linear mixed-effects regression analysis, ECV correlated well with the histological CVF in the DCM and ICM patients on a per-segment basis (β = 0.838 and β = 0.915, respectively, p < 0.001). In the multivariate linear regression analysis, histological CVF was the strongest independent determinant of ECV in the patients awaiting heart transplantation (standardised β = 0.860, p < 0.001). However, the T2 time, GLS, GCS and GRS showed no significant associations with ECV and CVF in the patients awaiting heart transplantation.ConclusionsECV derived from CMR correlated well with histological CVF, indicating its potential as a non-invasive tool for the quantification of myocardial fibrosis. Additionally, impaired myocardial systolic strains were not associated with the ECV and CVF in the patients awaiting heart transplantation.

The expression of ADAMTS-1 mRNA in myocardium of viral heart disease (VHD) mice was investigated to explore its role in myocardial fibrosis. A total of 150 purebred inbred Balb/c mice were used in this study. According to the principle of similar body weight, 50 mice were selected to make an acute viral myocarditis (VMC) animal model (acute VMC group), and 50 mice were selected to make a chronic VMC animal model (chronic VMC group), and the remaining 50 mice were selected as a control group. RT-qPCR was used to detect the relative expression of transforming growth factor-β1 (TGF-β1) mRNA and ADAMTS-1 mRNA in myocardial tissue of three groups of mice, and their relationship in myocardial fibrosis was analyzed. Compared with the control group, the collagen volume fraction (CVF) in the myocardial tissue of the acute VMC group was significantly increased, and the increase of CVF in the myocardial tissue of the chronic VMC group was the most significant (p<0.001). Compared with the control group, the relative expression of TGF-β1 mRNA and ADAMTS-1 mRNA in myocardial tissue of the mice in the acute and chronic VMC group were significantly increased (p<0.001). The relative expression of TGF-β1 mRNA and ADAMTS-1 mRNA in myocardial tissue of chronic VMC group was significantly higher than that of acute VMC group (p<0.001). Pearson's correlation test results showed that ADAMTS-1 mRNA was positively correlated with CVF and TGF-β1 mRNA, and the correlation coefficients were (r=0.351, p<0.01, r=0.401, p<0.01). ADAMTS-1 is involved in the occurrence and development of myocardial fibrosis, and it is positively correlated with CVF and TGF-β1. It may play a role in promoting myocardial fibrosis during the development of VHD. It can be used as a biological index for predicting myocardial fibrosis.

Las cardiomiopatías son un grupo de enfermedades de etiología primaria o secundaria, utilizadas por alteraciones en el músculo cardiaco (dilatación, hipertrofia, disfunción sistólica y/o diastólica), las cuales han cobrado notoriedad en los últimos años debido al desarrollo de nuevas técnicas de imagen. La resonancia magnética cardíaca es una técnica robusta, gold-standard para la evaluación de la morfología cardíaca y de la caracterización tisular, con muy poca variabilidad intraobservador e interobservador que brinda información muy útil permitiendo la identificación de la causa subyacente lo que permite brindar información no sólo pronóstica y de diagnóstico diferencial sino también para guiar el tratamiento y evaluar la respuesta del mismo.
