Background: Distal radius fracture is extremely common and represents 16% of fractures treated by orthopaedic surgeons. Near anatomical reduction with restoration of radial length, radial tilt and ulnar variances are important for good functional results. A variety of treatment options have been proposed for distal radius fracture closed reduction and immbolization in cast has been the main stay of treatment, but because it invariably results in malunion, poor functional outcome and cosmetic outcome, other modality of treatment were brought into practice like percutaneous intrafocal pinning, transulnar percutaneous pinning, external fixation, plating etc. This comparative study was to compare the clinical outcomes of closed reduction with cast and closed reduction with Percutaneous Kirschner wiring (PKW)/canullated cancellous screws (CC screws 4 mm)for the management of Colles fractures in patients between 20 and 70 years old. In this study we did an prospective study in 40 patients with extra articular distal radius fracture treated by both closed reduction with cast and closed reduction internal fixation (PKW/CC screws).Methods: There were 20 patients with Colles fractures treated by closed reduction with cast and 20 patients treated with closed reduction internal fixation (PKW/CC screws). We compared both the managements together. All patients were followed up in the orthopaedic department prospectively for at least 12 months between June 2015-June 2016 The functional outcomes and radiological results were compared between the two groups.Results: The study included 40 patients, aged between 20 to 69 yrs. Average follow up was 12 months. Using the demerit scoring system of Gartland and Werley we had excellent to good results of 60% in closed reduction internal fixation compared to closed reduction with casting excellent to good was 10%.Conclusions: The percutaneous pinning/cc screws and immobilization in neutral position for 3 weeks followed by physiotherapy proved to be better and simple procedure for extra articular non comminuted distal radius fractures.

Introduction: Distal radius fractures are common injuries encountered in orthopaedics casualty. Range of fracture extends from simple extra-articular to highly comminuted intraarticular fractures.1, 2 Restoration of wrist function is the primary goal in the treatment of extra articular distal radius fractures. Most of the fractures are effectively treated with closed reduction and immobilization in a plaster cast. Percutaneous pinning provides anatomic reduction and rigid fixation and allows earlier rehabilitation.3 Aim of the study was to assess the functional and radiological outcome of percutaneous pinning in distal radial fractures. Materials and Method: This is prospective study of 28 patients conducted at department of orthopaedics over a period of 2 years. Results: Age of the patients was ranged from 20 to 65 years with mean age of 40.2 years. Maximum incidence of injury was observed at 4 th (50%) and 3 rd (25%) decade of life. 20(71.43%) patients were male. 16 (57.14%) patients attained fracture due to motor vehicle accidents. Pin loosening was encountered in 5 cases. The mean radial length, radial volar tilt, and radial inclination were 10.24 mm, 8.26 degree, and 19.82 degree respectively. Functional outcome was excellent results in eight cases (28.57%), good in 13 cases (46.43%), fair in 5 (17.86%) cases and poor in 2 cases (7.14%).

Analysis of Distal Radius Fracture Treatment With an External Fixator

ÖZETThis study included 37 patients with closed distal radius fractures treated with an external fixator. Twenty-eight fractures were classified as type C and nine fractures were type B according to the AO classification system. Twelve months after the removal of the external fixator, the dorsiflexion angle measured 64.3° (45-75°), the supination angle measured 73.9° (62-85°), the palmar flexion angle measured 74.2° (60-85°) and the pronation angle measured 84.1° (78-90°). Twenty-four patients were evaluated as good, eight patients were evaluated as excellent and five patients were evaluated as medium-poor according to the Sarmiento modification. The mean score was 75.38 (55-90) according to the Mayo scoring system. The mean DASH score was 8.27. In 12 patients, Reflex Sympathetic Dystrophy (RSD) was occurred. Three patients in this group did not respond to medical and physical therapy. Three patients without daily life satisfaction were RSD patients. In 19 patients (51%), superficial pin tract infection occurred and was treated with antibiotics. There were no deep tissue infections. We concluded that the application of an external fixator for distal radius fractures led to satisfactory functional and surgical results. Bu kırıkların tariflenmesi klinik olarak 1814 yılında Colles tarafından yapılmıştır (1). Bu kadar çok karşılaşılmasına rağmen bugün bile tedavi seçenekleri tartışmalıdır (4). Kapalı redüksiyon ve alçı ile immobilizasyondan değişik cerrahi yöntemlere kadar çe-şitli tedavi yöntemleri tanımlanmıştır (4, 5). Bu yöntemlerin hepsi; radius uzunluğunu sağlamak, palmar tilt ve radial inklinasyonu düzeltmek ve radius eklem yüzünün distal anatomik dizilimini sağlamak amacıyla geliştirilmiştir (6, 7). Bu yazıda eksternal fiksatör uyguladığımız radius kırıklı 37 olgu bulunan serimizi literatür bilgileri eşliğinde sunduk. Olgu SunumuÇalışmaya 2003-2007 yılları arasında eksternal fiksatör ile tedavi edilen kapalı distal radius kırıklı (AO: Arbeitsgemeinschaft für Osteosynthesefragen sınıflaması; B ve C) otuz yedi olgu dahil edildi. Yirmi ikisi erkek, on beşi kadın olan hasta grubunun yaş ortalaması 47,38 idi. Kırıkların yirmi altısı dominant olmayan, on biri dominant tarafta idi. AO sınıflamasına göre yirmi sekiz olgu tip C (C1, dört; C2, on; ve C3, on dört olgu), dokuz olgu tip B (B1, iki; B2, üç; ve B3i dört olgu) olarak değerlendirildi.Kırıklara ilk kırk sekiz saat içerisinde cerrahi işlemleri gerçekleştirildi. Olguların hepsine aynı tip eksternal fiksatör uygulandı ve perkütan K telleri kullanıldı. El bileği nötral veya minimal palmar fleksiyonda sabitlendi. Ameliyatın hemen sonrasında falanks hareketlerine izin verildi. Tüm pansumanları poliklinik şartlarında yapıldı.
