External‐beam radiotherapy for differentiated thyroid cancer locoregional control: A statement of the American Head and Neck Society

Abstract: The use of external-beam radiotherapy (EBRT) in differentiated thyroid cancer (DTC) is debated because of a lack of prospective clinical data, but recent retrospective studies have reported benefits in selected patients. The Endocrine Surgery Committee of the American Head and Neck Society provides 4 recommendations regarding EBRT for locoregional control in DTC, based on review of literature and expert opinion of the authors. (1) EBRT is recommended for patients with gross residual or unresectable locoregiona… Show more

“…Schwartz et al [23] described their prescription of the following doses: the operative bed is treated to adjuvant doses of 57–60 Gy, microscopic positive surgical margins can be boosted to doses of 63 Gy, and gross disease is treated to doses of 66–70 Gy. Similar radiation dose recommendations were recently published by the American Head and Neck Society [1]. Additional investigation is necessary to define the optimal curative dose prescriptions for PTC [23].…”

“…However, it is unclear whether this is an endorsement of EBRT use in this setting. A recent statement by the AHNS recommends EBRT for patients with gross residual or unresectable locoregional disease, except in patients younger than 45 years old with limited radioiodine‐avid gross disease . The statement recommends against routine use of EBRT after complete resection of gross disease.…”

“…The most recent American Thyroid Association (ATA) guidelines recommend against routine use of EBRT after initial complete surgical removal of tumor due to lack of supporting evidence in the literature . Similarly, a recent statement by the American Head and Neck Society (AHNS) recommends against routine use of EBRT after complete resection of gross disease …”

“…Partial thickness resections of the esophageal wall, tracheal windowing with reconstruction, and partial laryngectomy are potentially valuable techniques permitting tumor resection without the morbidity of more extensive procedures or concerns surrounding microscopic residual tumor . Adjuvant external‐beam radiotherapy may be used in this setting …”
