Aim: To assess dietary intake and serum mineral and vitamin levels in elderly people from Northwest Spain consuming a Mediterranean diet, rich in vegetables, fruit, meat, fish, olive oil, dairy products and moderate in wine. Methods: Cross-sectional observational study in four retirement homes. Forty-five men and 65 women participated. Dietary intake and serum calcium, magnesium, iron, retinol, α-tocopherol, albumin, cholesterol, glucose and hematological parameters were determined. Results: Mean consumption of fruit plus vegetables was 600 g/day. Men consumed significantly more legumes, fruit, meat, and alcoholic beverages, but fewer vegetables than women. Women presented higher carbohydrate and lower alcohol energy contributions. Men consumed significantly more thiamin, niacin equivalents, riboflavin, retinol equivalents and iron. Vitamin D intake was 2.2 ± 1.2 µg/day and folate intake was 204 ± 47 µg/day without gender differences. Prevalence of anemia was 6.7% and that of high ferritin, 1.8%. Serum cholesterol and retinol were higher in women. Mean serum α-tocopherol/cholesterol ratio in the whole population was 6.3 ± 2.3 mmol/mol without gender differences. Conclusion: This elderly population consumes an Atlantic-Mediterranean diet that appears, according to biochemical and hematological parameters, appropriate. However, more fatty fish and leafy green vegetables and the inclusion of fortified foods in the diet might optimize micronutrient status.

Objectives: To examine folic acid and vitamin B 12 status in a group of 1000 persons sampled from the community of Umeå, Sweden, and aged 35, 40, 45, 50, 55, 60, 65, 70, 75 or 80 years. Reference data for folate and age-stratified reference data for vitamin B 12 are presented, together with an examination of potential confounders. Measurements: All subjects participated in extensive health examinations and interviews, and laboratory blood testing was performed. Results: A series of exclusion criteria were applied, and data from 961 subjects were analysed. Vitamin B 12 levels were found to decrease with increasing age, whereas folate levels remained constant across the age span studied. None of the vitamins was found to vary with sex, education, smoking or alcohol consumption, body mass index, prescription-free vitamin supplements, level of haemoglobin, or mean cell volume of erythrocytes. Further, none of these factors was associated with the agerelated decrease of vitamin B 12 level. Conclusions: The offered reference ranges should be used only in order to rule out deficiency. For B 12 levels, the age of the subject should be considered such that, for elderly people in particular, values above the medians should be considered as indicative of normal vitamin status.

SUMMARYObjective: To evaluate the nutritional state of older outpatients. Material and methods: In this case-series study, we evaluate a population of ambulatory elderly patients living in the community, using scales for functionality, a mini nutritional assessment, and anthropometrical measures. Results: In 66 patients, we found a mean age 73.3 ± 9.14 years old, females 68.2%, an average pathologies of 2.83 ± 1.06, drugs taken 1.10 1.26, mean IMC 25.74 ± 4.9, MNA score 20.72 ± 3.56, Katz, Lawton and Barthel scores 1.41 ± 1.08, 6.35 ± 2.24, 93.62 ± 18.18, respectively. The principal pathology diagnosed was cardiovascular (22.7%) and gastroenterological (16.7%). Searching for pathological states, we found a prevalence of protein-calorie malnutrition of 36%, the prevalence increasing with the age (from 19.2% in 60 to 69 years to 100% in 90 or more years). The prevalence of obesity was 42% in the general group decreasing from 73% in the age group of 60 to 69 y, to 13.3% in the age group 80 to 89 years. Using the mini nutritional 1 assessment scale, we found 51.5% of the population of patients with criteria for risk of malnutrition, and 19.6% with criteria of established malnutrition. Conclusion: We propose this measurements as practical and less expensive instruments for nutrition screening in older persons arriving to the examination room. Material y métodos: En este estudio de serie de casos, se evaluó una serie de pacientes adultos mayores ambulatorios viviendo en la comunidad, utilizando escalas de valoración funcional, mediciones antropométricas, y usando la escala llamada evaluación mínima nutricional. Resultados: La edad promedio de la población evaluada fue de 73.3 ± 9.14 años, 68.2% fueron del sexo femenino, el número de enfermedades promedio fue de 2.83 ± 1.06, el número de medicamentos consumidos promedio fue de 1.10 ± 1.27, el IMC promedio fue 25.74 ± 4.99, el score MNA promedio 20.72 ± 3.56. Los escores de Katz, Lawton, Barthel fueron en promedio 1.41 ± 1.08, 6.35 ± 2.24, 93.62 ± 18.18, respectivamente. La patología mas frecuente fue cardiovascular (22.7%), seguida por gastrointestinal (16.7%). La prevalencia de malnutrición proteíno-calórica fue de 36%, incrementándose con la edad (desde 19.2% en el grupo etáreo de 60 a 69 años, hasta 100% en el grupo de 90 y más) y la prevalencia de obesidad fué de 42% en el grupo general, decreciendo con la edad, desde 73% en el grupo etáreo de 60 a 69 años, hasta 13.3% en el de 80 a 89 años. Utilizando la escala de MNA, se encontró una población a riesgo de malnutrición de 51.5%, y con criterios de desnutrición establecida a 19.6% de la muestra. Conclusión: Nosotros proponemos estas mediciones como instrumentos de evaluación cómodos y poco costosos para valoración nutricional en pacientes acudiendo a consulta externa.
