Local delivery of cancer chemotherapeutics enables sustained drug levels at the site of action thereby reducing systemic side effects. A novel insertable polymeric drug delivery system for cervical cancer treatment is presented. Cisplatin, the first line of therapy employed for cervical cancers, was incorporated in a poly(ethylene-co-vinyl acetate) (EVAc) device that is similar to those currently used for vaginal contraceptive delivery. Cisplatin crystals were uniformly dispersed in the polymeric system without undergoing significant dissolution in the polymer matrix. Cisplatin dissolution from the devices was biphasic, consistent with a matrix-type controlled-release system with an initial rapid release phase followed by a slower, linear release phase. Depending on the drug loading in the polymeric devices, the near-linear release phase varied in rate according both empirical, linear curvefitting (0.38±0.15 μg/day to 46.9±10.0 μg/day) and diffusion analysis based upon diffusion through a porous structure (D app from 1.3±0.5×10 −9 cm 2 /s to 5.8±0.3×10 −12 cm 2 /s). The devices were tested for in vitro activity and found to be effective against both HPV positive and HPV negative cervical cancer cell lines. Preliminary studies indicate that this delivery system would be a good candidate for investigation as a choice of treatment in cervical cancers.

A working knowledge of contraception will assist the pediatrician in both sexual health promotion as well as treatment of common adolescent gynecologic problems. Best practices in adolescent anticipatory guidance and screening include a sexual health history, screening for pregnancy and sexually transmitted infections, counseling, and if indicated, providing access to contraceptives. Pediatricians’ long-term relationships with adolescents and families allow them to help promote healthy sexual decision-making, including abstinence and contraceptive use. Additionally, medical indications for contraception, such as acne, dysmenorrhea, and heavy menstrual bleeding, are frequently uncovered during adolescent visits. This technical report provides an evidence base for the accompanying policy statement and addresses key aspects of adolescent contraceptive use, including the following: (1) sexual history taking, confidentiality, and counseling; (2) adolescent data on the use and side effects of newer contraceptive methods; (3) new data on older contraceptive methods; and (4) evidence supporting the use of contraceptives in adolescent patients with complex medical conditions.

Gestagen alene og monofasiske p-piller, p-ring eller p-plaster kan brukes til forskyvning av menstruasjonen Ulike medikamenter, gestagen med eller uten østrogen, kan brukes til å regulere tidspunkt og hyppighet av en kvinnes menstruasjon. Monofasiske p-piller, p-plaster eller p-ring er alle velegnede midler, også der kvinnen har behov for langvarig eller hyppig forskyvninger av menstruasjonen. Gestagenet noretisteron er et alternativ ved kortvarig utsettelse av menstruasjonen.Mange kvinner ønsker å ha kontroll over menstruasjonsblødningene og å kunne forskyve dem etter ønske (1). Det kan også vaere medisinske grunner til å ville forlenge sykluser, slik som ved endometriose, dysmenoré, migrene og andre menstruasjonsrelaterte plager (2). Kortvarig forskyvning av menstruasjonen i forbindelse med ferie, eksamen og reise har vaert gjort i mange år. I idretten er langvarig forskyvning av menstruasjonen velkjent. Artikkelen omtaler bruken av de tilgjengelige medikamentene for forskyvning av menstruasjon.Artikkelen er basert på et søk i PubMed med søkeord «postponement», «menstruati-on», «withdrawal bleeding» og «avoiding menstruation» i ulike kombinasjoner. Det er gjort et skjønnsmessig utvalg av artikler basert på forfatternes erfaringer innen feltet. Informasjon er også hentet fra legemiddelhåndboken.no og legemiddelverket.no. FysiologiMenstruasjon kommer fra latin og betyr «månedlig». Den er et uttrykk for en fysiologisk syklus hos kvinnen. Normal menstruasjonssyklus er 21-35 dager, hvor tid etter eggløsning, lutealfasen (sekresjonsfasen), er temmelig konstant. De fysiologiske prosessene som utløser menstruasjonen begynner to dager før forventet menstruasjonsblødning. Progesteron hemmer østrogenin-dusert proliferasjon av endometrieepitelet og gir et sekretorisk endometrium ved forutgående adekvat østrogenstimulering. Ved et brått fall i progesteron-og østrogenkonsen-trasjonen forårsakes konstriksjon i arteriolene i endometriet og lokal iskemi som fører til avstøtning av endometriet (3) (fig 1). Ved hjelp av medikamenter kan man forskyve menstruasjonen kortere eller lengre tid. GestagenMenstruasjonen kan utsettes ved bruk av et gestagen. Gestagener er stoffer som er syntetisk fremstilt og har progesteronliknende effekt. De brukes i prevensjonsmidler, hormonell substitusjonsbehandling, ved dysfunksjonelle menstruasjonsblødninger og til menstruasjonsforskyvning (4).Ved menstruasjonsforskyvning med gestagener bør kvinnen utelukke at hun er blitt gravid i siste syklus. Utsettelse av menstruasjonen med gestagener medfører ingen risiko under amming (4). Noretisteronacetat, gestagenet som brukes til menstruasjonsforskyvning, har en stabiliserende effekt på endometriet. Det hemmer sekresjon av gonadotropinene luteiniserende hormon (LH) og follikkelstimulerende hormon (FSH), og i tilstrekkelig dose hemmes ovulasjonen. Vanlig dosering av noretisteron er 5 mg tre ganger daglig. Dersom behandling startes minst tre dager før forventet menstruasjon, altså før avstøtningsprosessen vanligvis igangsettes, vil menstruasjonsblødningen ...
