A pikkelysömör kialakulásában és progressziójában a betegek és az orvosok jelentős hányada a stresszt tartja az egyik legfontosabb rizikótényezőnek. Mivel a stresszor és hatásai is egyaránt szubjektumfüggőek és így nehezen mérhe-tőek, sokáig csupán kevés, a stressznek a betegség kialakulásában játszott szerepét vizsgáló tanulmány készült. Az elmúlt években azonban egyre több nívós, objektív mérésekkel alátámasztott vizsgálat született, amelyek új, érté-kes adatokkal szolgáltak a háttérben zajló patofi ziológiai folyamatok megértéséhez. A szerzők a szakirodalomban fellelhető stresszexpozíciót és -percepciót befolyásoló pszichológiai (alexithymia, személyiség, hangulat) és a stresszválaszt szabályozó biológiai (kortizol-, adrenalinszintek, neurogén gyulladás) tényezőket elemző vizsgálatokat integratív, pszichoszomatikus szemlélettel tekintik át, és bemutatják, hogy az eddig főleg csak tapasztalati szinten bizonyí-tott kapcsolatot egyre több modern, magas színvonalú kutatási eredmény igazolja. Ismerve a stresszcsökkentő pszichofarmakológiai és pszichoterápiás intervenciók sikerességét a pikkelysömör kezelésében és a betegek életminő-ségének javításában, a szerzők bíznak abban, hogy tanulmányuk hozzájárul a pszichoszomatikus szemlélet szélesebb körű elterjedéséhez a mindennapi bőrgyógyászati gyakorlatban. As both stressor and its effects are subject-dependent, thus extremely diffi cult to measure, our understanding of the exact role of stress in disease development was limited for a long time. In the past decade several new studies were carried out which expanded our knowledge on the pathophysiologic processes linking stress to psoriasis via with their objective measurements and the applied new techniques. The authors review the current literature of both psychological (alexithymia, personality, affect) and biological (cortisol, epinephrine, neurogenic infl ammation) factors infl uencing stress perception and response in psoriasis. Results of recent investigations support previous reports about the interaction between stress and psoriasis with objective evidence. Knowing how effective stress-reducing psychopharmacologic and psychotherapeutic interventions are in the treatment of psoriasis the authors hope that this review contributes to a wider acceptance of the psychosomatic attitude in everyday dermatologic practice.

Pruritus is a frequent but often unrecognized and underestimated symptom in patients with psoriasis. The underreporting may be due to lower frequency and reduced intensity of pruritus compared to other pruritic diseases such as atopic eczema or uremic pruritus, as well as impairment of psychosocial well-being caused by e.g. depression, social withdraw and secretiveness. Recent studies show pruritus being more prevalent in psoriasis, especially plaque-type psoriasis, than previously believed and severely affecting patients' quality of life. The assessment of pruritus and its associated effects is a significant component of clinical management of psoriasis. The presented suggest we should not underestimate pruritus in psoriasis and must consider this symptom in planning therapy.

Bacterial biofilms constitute a critical problem in hospitals, especially in resuscitation units or for immunocompromised patients, since bacteria embedded in their own matrix are not only protected against antibiotics but also develop resistant variant strains. In the last decade, an original approach to prevent biofilm formation has consisted of studying the antibacterial potential of host communication molecules. Thus, some of these compounds have been identified for their ability to modify the biofilm formation of both Gram-negative and Gram-positive bacteria. In addition to their effect on biofilm production, a detailed study of the mechanism of action of these human hormones on bacterial physiology has allowed the identification of new bacterial pathways involved in biofilm formation. In this review, we focus on the impact of neuropeptidic hormones on bacteria, address some future therapeutic issues, and provide a new view of inter-kingdom communication.

Abstract: Tachykinins may play a role in psoriasis per se, in addition to pruritus in this disease. Targeting the combined NK-1 and NK-2 receptors might be a possible treatment.
