Exploring the relationships between International Classification of Functioning, Disability and Health (ICF) constructs of Impairment, Activity Limitation and Participation Restriction in people with osteoarthritis prior to joint replacement

Abstract: BackgroundThe International Classification of Functioning, Disability and Health (ICF) proposes three main constructs, impairment (I), activity limitation (A) and participation restriction (P). The ICF model allows for all paths between the constructs to be explored, with significant paths likely to vary for different conditions. The relationships between I, A and P have been explored in some conditions but not previously in people with osteoarthritis prior to joint replacement. The aim of this paper is to exa… Show more

“…Pain is generally considered to be an impairment measure as well 300 but in contrast to ROM correlates well with measures of activities limitations and participation restrictions. We would argue that it may be inappropriate to classify pain as an impairment measure in this context.…”

“…Research has shown that the relationship between the impairment, activities limitations and participation restrictions domains of the ICF are not simple, with other factors such as self-efficacy and comorbidities acting as independent determinants of the relationships between these variables. 300 It has been recommended that a combination of outcome measures should be used to assess function after total hip or knee replacement. 301,302 However, there are many reasons not to use a wide number of measures with every patient, whether in clinical practice or research.…”

“…These concepts have been criticised in the literature for not being conceptualised to the depth needed, and not being adequately differentiated [6,17]. However, some empirical studies have demonstrated that the two concepts are distinct and recommended detailed and separate assessment of each [18][19][20].…”

“…When selecting outcome measures, it is important to ensure that outcomes measure the specific construct and do not overlap other constructs in the same theory [29]. If there is poor content validity and the measures used in a study do not discriminate among constructs, any…”
