Exploring construct validity of clinical staging in schizophrenia spectrum disorders in an acute psychiatric ward

Abstract: this study demonstrates that clinical staging in schizophrenia spectrum disorders has an acceptable construct validity between earlier and more chronic stages of disease. Several clinical profilers increase in more advanced stages compared to earlier clinical stages, which supports construct validity.

“…Earlier reports regarding variation in symptom severity between clinical stages provided variable results. Two studies reported higher index severity scores of negative symptoms and cognitive decline in stage 4 compared to earlier stages (Berendsen et al, 2018;Godin et al, 2019). On the other hand, Tedja and colleagues found that baseline stages were not associated with negative symptoms at follow-up and cognition was primarily not affected (Tedja et al, 2017).…”

“…In recent years, clinical staging of schizophrenia spectrum disorders has received increased attention in scientific literature. Three exploratory validation studies of the staging model in schizophrenia demonstrated that more advanced stages of disease have higher severity scores at symptom level and a wide variety of clinical profilers (Berendsen et al, 2018;Godin et al, 2019;Tedja, Velthorst, van Tricht, de Haan, & Group, 2017). More specifically, two of these studies suggested that advanced disease stages have higher severity scores of depression, cognitive decline and negative symptoms.…”

“…Future studies should take into account the fact that schizophrenia is diagnosed based on a sum of different clinical criteria, and this could lead to different types of schizophrenia with different biomarkers. Moreover, in schizophrenia different clinical stages have been described 42,43 that could be based in different neurobiological alterations, and this could affect the retinal parameters. Finally, regarding this disease, some studies suggest that it is a neurodevelopment illness 44,45 while others support a neurodegenerative component (Gupta et al, 2009).…”

“…In this regard, the recently developed models based solely on the Positive and Negative Syndrome Scale (PANSS) deserve a separate mention [11][12][13]. Additionally, we have noticed a growing interest in validating some of the proposed theoretical models [6] for the purpose of establishing their validity and/or improving them [14][15][16][17][18][19][20][21][22]. However, despite these above-mentioned efforts, practically all of these models have significant limitations [6].…”
