Experimental High‐Volume Hemofiltration With Predilutional Tris‐Hydroxymethylaminomethane for Correction of Low Tidal Volume Ventilation‐Induced Acidosis

Abstract: The most common method of controlling acidemia during lung-protective ventilation is CO₂ removal with an extracorporeal lung assist (ECLA) system. Another possibility to prevent acidemia is based on intravenous (i.v.) application of tris-hydroxymethyl-aminomethane (3 mol/L, THAM) buffer, which can bind hydrogen protons and which can be removed from the body via renal replacement therapy (RRT). We investigated whether RRT combined with predilutional (prefilter) THAM-application provides an alternative to ECLA f… Show more

“…Permissive hypercapnia and acidosis in favour to lung protective ventilation strategies is a very dominant regime overruling various other therapeutic aspects. The option to manage nutrition could become a further argument to consider new ideas [43] to enable both, lung protection and normal pH homeostasis.…”

“…Another group removed bicarbonate by using pre-filter replacement fluid containing THAM. Physiologic CO 2 levels and pH were maintained for 1.5 hours, but it was not determined whether THAM had the same long-term problems seen in the hemodialysis models [67]. Nonetheless, respiratory dialysis holds much promise if the problems of electrolyte and acid-base disturbances can be solved.…”

“…Martin Russ et al. (59) of Charité‐Universitätsmedizin Berlin, Campus Virchow, Berlin, Germany investigated whether renal replacement therapy combined with predilutional (prefilter) tris‐hydroxymethyl‐aminomethane (THAM) provides an alternative to extracorporeal lung assist for a rescue situation. Six pigs underwent 5 h of acidemia and permissive hypercapnia.…”
