Experimental diabetes induces functional and structural changes in the peritoneum

Abstract: These data demonstrate that chronic hyperglycemia is associated with functional and structural changes in the peritoneum that parallel with selective regulation of NOS isoforms and AGE deposits. The alterations are prevented by insulin treatment, which suggests that adequate control of diabetes can preserve PM integrity in diabetic patients prior to PD.

“…5,7,8,[15][16][17]27,28 Previous studies have found no significant association between peritoneal wall thickness by US and peritonitis frequency. 15,[17][18][19] On the contrary, however, we found for the first time, a positive correlation between peritoneal thickness and episode of peritonitis (R 2 ¼ 0.933, Figure 4C).…”

“…Although the definitive diagnosis of PFS can be made by peritoneal biopsy, ethical issues, difficulty of the process and insufficient knowledge of PFS are the major obstacles for routine clinical application of peritoneal membrane biopsy. 3,[5][6][7][8][9][10] Measurement of cytokines levels which are responsible in PFS progression can be an alternative diagnostic tool. 3 However, there is no consensus on the time and the frequency of cytokines monitoring in the diagnosis of PFS.…”

“…Наиболее вероятной причиной этих изменений яв-ляется постоянный контакт брюшины с растворами с высоким содержанием глюкозы. Аналогичные изменения в брюшине вы-зывают гипергликемия и поздние продукты гликирова-ния [43,44]. Таким образом, уровень глюкозы у больных на перитонеальном диализе должен быть приближен к физио-логическому уровню настолько, насколько это возможно.…”

“…Glucose, glucose degradation products, and advanced glycation end-products are considered to be the most important factors (7)(8)(9)(10). Those findings suggest that peritoneal damage at the start of PD may be more severe in patients with diabetes than in those who are diabetesfree.…”
