Annotation. The study aimed to analyze references in the scientific literature on hard dental tissue damage and periodontal and oral mucosal diseases in patients with concomitant gastrointestinal tract pathology. We used the bibliosemantic method to identify the status of the outlined issues and analysis of the scientific research data. Based on data from national and international published literature on the topic, the prevalence, severity and structurality of changes in the organs and tissues of the oral cavity associated with gastrointestinal tract diseases were evaluated. The review was carried out using data from scientific articles for the period of 2011-2022 with search words “Digestive System Diseases”, and “Diseases of the Oral Cavity” using the MESH subject headings: dental caries susceptibility, mouth diseases, gastrointestinal diseases, malabsorption syndromes, dental caries, tooth demineralization and others. The final analysis included 75 scientific articles corresponding to the purpose of the study. Data obtained from analysis of literary sources of recent years suggests that both local and general factors are involved in the process of occurrence and course of dental pathology with concomitant involvement of the gastrointestinal tract. The nature of their interaction will determine the peculiarities of the course of dental diseases. However, the lack of convincing data on the age-related features of the occurrence and development of dental diseases, as well as the inconsistency of the results regarding the significance of risk factors for the progress of comorbid conditions complicates the methods development of their treatment and prevention. The listed features of the course of dental diseases with a combined involvement of gastrointestinal disorders indicate the mandatory complexity of treatment, taking into account concomitant pathology.

Aim: To evaluate the therapeutic effectiveness of low-intensity laser irradiation in the treatment of patients with oral mucosal diseases in association with rheumatoid arthritis, according to parameters of Doppler ultrasonography of blood flow of lingual arteries. Materials and Methods: An examination of 25 patients with geographic tongue and concomitant rheumatoid arthritis and 12 practically healthy persons of the control group was carried out. In the main group (13 patients) the complex treatment of glossitis was carried out with non-contact laser irradiation of the affected tongue areas. Patients in the comparison group (12 persons) underwent standard treatment. Treatment effectiveness of patients in both groups was determined by the parameters of Doppler blood flow study of lingual arteries. Results: In the main group of patients, after diode laser therapy, a significant decrease in linear blood flow parameters (Vmax by 19.7%, TAMAX by 19.6%), an identical increase in volume velocity (Q by 21%) and an improvement in vascular tone and peripheral resistance (Pourcelot index by 12.5%, Gosling index by 13.1%) were observed. At the same time, a slight increase in the linear and volume velocity characteristics of blood circulation in lingual arteries during treatment of patients in the comparison group was not statistically significant (p>0.05) and with a high degree of probability differed from the control group. Conclusions: The use of low-intensity laser irradiation in the management of patients with oral mucosal diseases associated with rheumatoid arthritis contributes to the normalization of hemodynamics and the functional state in the vascular bed of the tongue.

Aim: To compare the effectiveness and safety of various methods of professional teeth bleaching using light and chemical activation systems in an in vivo study. Materials and Methods: Teeth whitening was performed on 48 patients aged 20 to 35, divided into 4 groups. In patients of group 1 for dental bleaching a diode laser activation was used, group 2 – LED/gel activation, group 3 – chemical whitening with the following remineralization therapy, group 4 – chemical whitening without remineralization therapy. The study of the effect of bleaching systems on the hard tooth tissues was carried out by index assessment, determination of biochemical parameters of an oral fluid. Results: Laser bleaching of the vital teeth was significantly more effective than the other methods with 95% confidence level and did not affect the structure of hard dental tissues and mineral exchange of the oral fluid. LED irradiation has a high whitening effect, but leads to dentin hypersensitivity and a decrease in phosphorus and alkaline phosphatase of oral fluid. Chemical bleaching causes a decrease in enamel resistance, the occurrence of tooth sensitivity, violation of mineral exchange of oral fluid, therefore; it requires combined use with local remotherapy. Conclusions: Laser teeth bleaching is the effective and safe method in management of discolored teeth. A positive result of its use can be considered an improvement of color change of the teeth by 3.33 shades compared to chemical bleaching, enamel stability, low values of dental hypersensitivity indices, no changes in biochemical parameters of the oral fluid.
