Expenditures and Health Care Utilization Among Adults With Newly Diagnosed Low Back and Lower Extremity Pain

Abstract: IMPORTANCE Low back pain (LBP) with or without lower extremity pain (LEP) is one of the most common reasons for seeking medical care. Previous studies investigating costs in this population targeted patients receiving surgery. Little is known about health care utilization among patients who do not undergo surgery. “…Between 2000 and 2008, there was a marked increase in epidural injections for management of chronic pain in the Medicare population, with a 108% increase in the number of patients receiving any spinal injection and a 253% increase in the number of epidural injections specifically [62]. While utilization of epidural injections for management of chronic pain decreased from 2009-2018 [63], back pain with or without radicular pain remains a major reason for disability and loss of productivity [64]. Generally, patients with spinal stenosis, bilateral radicular pain, and/or multi-level degenerative disc disease receive an inter-laminar epidural (Fig.…”

“…Po troškovima lečenja u Americi, na LBP se troše milijarde dolara (3). Pacijenti koji ne odlaze na hiruršku intervenciju, često dobijaju terapiju koja nije konzistetntna sa kliničkim vodičima (4). Većina bolesnika sa akutnim lumbalnim bolom se leči kod lekara primarne zravstvene zaštite, koji treba da ih informiše o dobroćudnoj prirodi bolesti i dobroj prognozi.…”

“…Due to this absence of underlying disease, healthcare professionals find the clinical assessment and treatment of such patients extremely difficult [3]. The use of healthcare services and medication intake is not only higher in patients with chronic pain than in the general population, but is even higher in those with spinal pain [4]. Thus, health service funders and patients experience increased costs [5].…”
