Expanding Ambulatory Care Pharmacy Residency Education Through a Multisite University-Affiliated Model

A core component of the vision of the American College of Clinical Pharmacy (ACCP) for the profession is that most pharmacists will provide direct patient care. Residency training programs are considered a prerequisite to meet the standards and competencies required for pharmacists providing direct patient care. Barriers to residency training expansion are numerous and include, but are not limited to, lack of funding, challenges in meeting accreditation standards for residency programs, limited practice sites, and lack of qualified preceptors. The demand for postgraduate year one (PGY1) and postgraduate year two (PGY2) pharmacy residency training has exceeded position availability over the past 10 years. The American Society of Health‐System Pharmacists (ASHP) Foundation Pharmacy Forecast 2021 predicts pharmacy expansion within ambulatory care, population health, specialty pharmacy, and home care. To promote pharmacy residency expansion and maintenance of pharmacy residency quality, this commentary makes four recommendations with specific actions for consideration: develop preceptors and promote board certification, promote and foster innovation in residency training, expand residency training to address health equity with a focus on rural/underserved areas, and expand nontraditional residency training programs.

Enhancing resident–preceptor relationships through resiliency discussions

Background/Purpose: Studies suggest utilization of support networks and mentors is a resiliency-building strategy for health professional students. Likewise, a supportive resident-preceptor relationship has been associated with increased resident wellness. Despite this information, the impact of resident learners specifically connecting on resiliency topics with their preceptors is limited in the literature. The purpose of this study is to determine residents' perceptions of the impact of resiliency debriefing sessions with their preceptor.Methodology: Nine 1-h resiliency sessions were designed and delivered to 27 residents. Topics included: Introduction to Wellbeing, Balance, and Support Systems; Enneagram; Self-Care; Imposter Syndrome; Brene Brown Critics and Feedback; Simon Sinek's "Start with Why"; Mindfulness and Gratitude; Mindset; and Creating a Mantra. After each resiliency session, residents debriefed with their clinical preceptor(s) within 1 week, which included a session content overview, perceived key takeaways, and what residents wanted more information on. Residents were then asked to complete a nine-question survey assessing their perceptions.Results: After delivery of nine resiliency sessions and preceptor debrief, 112 (46%) survey responses were recorded. The majority of residents (72%) agreed or strongly agreed that discussing these topics with preceptors was useful. Over half of residents felt preceptors were able to provide additional resiliency tools or resources (56% agreed or strongly agreed) and mentoring (63% agreed or strongly agreed). Evaluating the evolution of philosophy of practice over postgraduate year 1 residency training

Introduction: Philosophy of practice (PoP) serves as the foundation for any patient care practice. It is a set of professional values and beliefs that guides a practitioner's actions and decisions in practice. The objective of this study was to evaluate if and how postgraduate year 1 (PGY-1) residency training changed pharmacy residents' PoP.Methods: This was a qualitative comparative analysis of PGY-1 residents' PoP at the start vs the end of residency. In July 2019, 23 incoming ambulatory care PGY-1 residents at the University of Minnesota were asked to articulate their PoPs. In April 2020, the same cohort was asked to revisit their PoPs and make any necessary revisions. Baseline PoPs were coded inductively to create a standard codebook. The codebook was applied to final PoPs to assess any changes or new codes that emerged.Results: Three new codes emerged in the final PoPs: improving health of the community, educating the future generation of pharmacists, and professional advocacy and service. One of the largest differences that occurred was the frequency of the code "interprofessional team collaboration," with only five residents including this in their baseline PoP compared with 16 in the final PoPs. Conclusion:After 1 year of residency training, concepts such as interprofessional collaboration became more prevalent in residents' PoPs suggesting this may be a concept residents understand and appreciate more during residency. Additionally, the emergence of new codes, such as professional advocacy and service, may also reflect the professional identity development that occurs during residency.
