Exercise-induced Bronchospasm In Children

Abstract: This review will encompass definition, history, epidemiology, pathogenesis, diagnosis, and management of exercise -induced bronchospasm in the pediatric individual with and without known asthma. Exercise induced asthma is the conventional term for transient airway narrowing in a known asthma in association with strenuous exercise usually lasting 5-10 minutes with a decline in pulmonary function by at least 10%. Exercise induced asthma will be referred to as exercise induced bronchospasm in an asthmatic. Exerci… Show more

“…Together with the frequent nocturnal awakenings due to dyspnea, EIAO may hamper social contacts. Exercise is a common trigger of bronchial hyperresponsiveness and may cause cough, wheezing, and chest tightness [63, 64]. EIAO is indicative of insufficiently controlled asthma [65].…”

“…With the high ventilation rates associated with higher oxygen requirements during vigorous exercise, and the increased role of mouth breathing, the warming and humidification function of the upper airways is inadequate or bypassed. Cold and dry air can cause changes in osmolality in the airway epithelium, which leads to an inflammatory state (5,6) that involves the release of proinflammatory cytokines or mediators, including chemokines, histamine, or leukotrienes. The cold air can also stimulate cholinergic receptors, which may lead to an added bronchoconstrictive effect.…”

“…Subsequently the terms “EIA” and “exercise-induced bronchospasm” (EIB) are often used interchangeably 2. However, EIB specifically denotes the reduction in lung function after a standardized exercise test 1,3,4. “Exercise-induced bronchoconstriction” is another term that has been used in the literature 5.…”
