Exenatide as a Novel Weight Loss Modality in Patients Without Diabetes

Abstract: Obesity continues to be a national epidemic, while choices for effective pharmacologic treatments are extremely limited. Exenatide appears to have promising effects on weight in overweight or obese adults without type 2 diabetes. Further investigations with large, placebo-controlled trials assessing long-term weight loss as a primary outcome are warranted.

“…These data are consistent with the concept that an effect of exenatide on gastric emptying may contribute to the weight loss observed in larger, longer duration, randomized, placebo‐controlled trials (Moreno et al. ). These results are also consistent with effects of exenatide on gastric emptying which are temporally associated with reduced postprandial glycemia in patients with type 2 diabetes mellitus (Linnebjerg et al.…”

“…However, stimulation of short-term, episodic signals represents a promising therapeutic option, as small clinical studies with exendin-4 have shown a potential for sustained weight loss in obese individuals [262], and even more promising may be multi-hormone "cocktail" treatments, which target both episodic and tonic signals to bring about weight loss [263]. Similarly, newly developed PEGylated peptide analogues, such as (pGlu-Gln)-CCK-8 with increased enzymatic stability that impede rapid degradation are also promising therapeutic options for obesity [264].…”

“…Both liraglutide and exenatide twice/day have been evaluated for weight loss in patients without diabetes. A systematic literature review published in 2012 identified five studies examining the effects of exenatide on weight in patients without diabetes . Patients enrolled in these studies were overweight or obese, with a family history or other indications of prediabetes.…”
