Excision of Tumours of Humerus and Femur, With Restoration by Internal Prostheses

Abstract: A personal experience is recounted of operations in cases of tumour involving the humerus or femur with restoration by endoprostheses. Twenty-four patients were treated in this way from 1950 to 1969 inclusive and have been followed up for between four and twenty-four years. The patients selected for treatment have presented chondrosarcoma (ten), so-called benign giant-cell tumour of bone, usually recurrent (nine), angiomatous osteolysis (two), seemingly single thyroid or renal metastasis (two), and unumted pat… Show more

“…5,8,10,13,14,17,26,33 Several reports suggest tumors of bone, including chondrosarcomas that arise in the proximal humerus, are more difficult to treat than other sites because of the proximity of vessels and nerves. 3,4,6,7,11,12,16,27,30,31,34,35 The muscular attachments to the humerus at the shoulder and the alterations in shoulder stability resulting from resection make this site a more complex problem in reconstruction. All reconstruction approaches, including fibular transplant, metallic implants, arthrodesis, and allograft reconstruction, may result in functional problems for the patient.…”

“…Segmental endoprosthetic replacement has gained popularity among orthopaedic oncologists as an expedient and effective means of addressing large segmental proximal femoral deficiencies after excision of primary metastatic tumors (1,4,14,17). In most large series, complication rates of these reconstructive methods have approached 15-20%, with radiographic evidence of loosening a common occurrence (1 JOJ 4,15,17).…”

“…1 In the USA, Neer 2 developed a stemmed unconstrained humeral prosthesis specifically for the treatment of four-part fractures. His design was successful in providing a scaffold around which the proximal humerus could be rebuilt.…”
