Most children with tumors will require one or more surgical interventions as part of the care and treatment, including making a diagnosis, obtaining adequate venous access, performing a surgical resection for solid tumors (with staging and reconstruction), performing procedures for cancer prevention and its late effects, and managing complications of treatment; all with the goal of improving survival and quality of life. It is important for surgeons to adhere to sound pediatric surgical oncology principles, as they are closely associated with improved local control and survival. Unfortunately, there is a significant disparity in survival rates in low and middle income countries, when compared to those from high income countries.The International Society of Paediatric Surgical Oncology (IPSO) is the leading organization that deals with pediatric surgical oncology worldwide. This organization allows experts in the field from around the globe to gather and address the surgical needs of children with cancer. IPSO has been invited to contribute surgical guidance as part of the World Health Organization Initiative for Childhood Cancer. One of our goals is to provide surgical guidance for different scenarios, including those experienced in High-(HICs) and Low-and Middle-Income Countries (LMICs). With this in mind, the following guidelines have been developed by authors from both HICs and LMICs. These have been further validated by experts with the aim of providing evidence-based information for surgeons who care for children with cancer.We hope that this initiative will benefit children worldwide in the best way possible.

“…Although complete resection was achieved in 13 patients, morbidity was significant in 6, including renal ischemia, common bile duct injury, diaphragm injury and vascular injuries including a portal vein injury. Qureshi et al reported 15 patients (9 mature and 6 immature teratomas), of whom 10 underwent surgery in infancy (16). Two patients with large midline immature teratomas extending into the root of the mesentery underwent partial resection and chemotherapy without appreciable benefit.…”

Germ cell tumors in infants are most frequently extragonadal, benign, and amenable to surgical resection. An unusual feature of germ cell tumors is the potential coexistence of malignant with benign disease which makes it possible for patients with incompletely resected tumors to experience either a benign or malignant recurrence. A challenge to postoperative surveillance is the interpretation of serum alpha fetoprotein, a marker of malignancy, that is physiologically elevated during the first year of life. A rare subset of germ cell tumors occur in the retroperitoneum. Although the vast majority are benign, these tumors are often large and distort normal anatomy, and may demonstrate local invasiveness that increases risk of resection. The intent of these reports is to caution readers about these unusual features of germ cell tumors of infancy.

Antecedentes: Los teratomas representan el 3 % de las neoplasias malignas infantiles y son el tercer tumor retroperitoneal pediátrico más común. Reporte de caso: Femenina de 4 años que fue ingresada por dolor abdominal, distensión y masa intrabdominal palpable. En su abordaje la tomografía evidencia una masa compatible con teratoma, por lo que se realiza laparotomía exploradora donde se observa tumoración con múltiples adherencias, se logra resecar por completo sin complicaciones, cursa su postoperatorio sin incidentes y es egresada a los 5 días. La histopatología confirmó teratoma maduro. Conclusiones: El teratoma retroperitoneal a pesar de su poca incidencia, es una patología que verá cualquier medico de primer contacto inicialmente y se debe tener en cuenta en el diagnóstico diferencial ante un paciente con masa intraabdominal y dolor abdominal. Su manejo oportuno es imperativo.
