Excision of carbuncle with primary split‐thickness skin grafting as a new treatment modality

Abstract: Carbuncles are debilitating skin infections commonly seen in diabetic patients. Excision of these infective lesions leads to large defects that require prolonged hospital stay and repeated dressings with ensuing pain and bleeding. This study is an attempt to cover the wounds resulting from excision of carbuncle with primary skin grafting so as to decrease the hospital stay and frequency of dressings.

“…1 It is most commonly caused by Staphylococcus aureus that usually starts as a furuncle/ boil around the root of a hair follicle. [2][3][4][5] On a literature search by the authors, carbuncles with such extensive involvement of the neck region have not been reported thus far and to the best of our knowledge this case was the first reported. Whereas poor hygiene, advanced age, an impaired immune system and delay in seeking medical advice seem to be the most important factors in such an extensive spread.…”

“…While skin grafts or flaps are the recommended techniques for coverage of large carbuncle defects not amenable to direct surgical closure. 5,6 The results of continued daily dressings and good sugar control may be quite surprising as evidenced by the remarkable wound healing that occurred in this case. An option of management that was undertaken solely on the behest of the patient's attendants.…”

“…As it enlarges, blood supply to the central part gets disrupted and leads to central necrosis. This may be seen in patients who have diabetes, immunodeficiencies, obesity, or sit for a prolonged period of time 3,4 . Most of the carbuncles are caused by methicillin resistant S. aureus, nonresistant staphylococci and streptococci and rarely by the Granulicatella adiacens 5 .…”

“…Early debridement and proper antimicrobial drugs give adequate treatment for carbuncle 6 . Excision of these infective lesions may lead to large defects prolonging hospital stay 4 . Wound care after cutaneous surgery can play an integral role in wound healing 7 and often heal with residual scarring.…”
