Screw metal implants with rough or the smooth polished were introduced surface in a tibial bone proximal condyle of not purebred rabbits. Within 6 months after operation the considerable distinctions of radiological data were revealed not. 2 months later after introduction of implants with a rough surface the effort enclosed for its twisting is, much more, than for removal of the polished product. However, stability of fixing of implants was practically made even at 6 months. On remote rough implants there is a set of tissue scraps whereas on products with a smooth surface the tissue remains were much less. Surrounding tissues strongly join a rough surface, grow into cavities, and during removal of such products there is a considerable trauma of tissues round an implantation place. Smooth implants have the smaller area of contact with organism tissues, they are fixed due to bicortical implantation, during removal easily get out and don't break off surrounding tissues.

Introduction Hormonal therapy and gender-conforming surgeries are the treatments of choice in appropriately selected male-to-female transsexuals. The Prosthetic Augmentation Mammoplasty (PAM) in transwomen is the second most common sex conforming procedure performed in our department. Aim This study was aimed to assess the esthetic outcomes of PAM performed from 2007 to 2017 in 42 transwomen. Patients and Methods It is a retrospective study wherein the documents of 42 transwomen who had undergone PAM were reviewed. Twenty-three transwomen who had pinch fold thickness more than 2.5 cm had undergone subglandular/subfascial (SG/SF) plane augmentation; rest had dual plane augmentation. All the cases were followed-up for an average period of 45 months. A validated institutional score for subjective assessment and objective assessment (by two independent observers) were used at the end of follow-up period. Statistical Analysis Used Student’s t-test was used for statistical analysis. Results On an average, 92.85% of transwomen achieved grade-A score with both subjective and objective assessment scoring system. Conclusion PAM gives a finishing and colossal touch for transwomen to orient their somatic sex to psychic sex. To get the best cosmetic outcomes and lasting results the PAM in transwomen has to be made a more reasoning and scientific procedure rather than an emotional and irrational procedure. Author’s PAM approach facilitates transwomen to rejoice their veritable total genderness in harmony with their gender identity.

RESUMOO transexualismo masculino é uma condição que exige a atuação de profissionais de diversas áreas para o diagnóstico e tratamento. De vital importância é o correto diagnóstico, uma vez que o tratamento cirúrgico é irreversível e, se incorretamente indicado, pode levar até ao suicídio. Os elementos diagnósticos são essencialmente clínicos e um período-teste de observação de dois anos é recomendado antes da realização da cirurgia. Nesse período são utilizados recursos psicoterápicos e prescrita medicação anti-androgênica e estrogênica para adequação dos caracteres sexuais secundários. No presente artigo de revisão são abordados os conceitos necessários à conduta nos casos de transexualismo, bem como as opções terapêuticas disponíveis. ABSTRACTMale transsexualism is a condition involving the cooperation of multiple specialists for diagnosis and treatment. Correct diagnosis is crucial once the surgical treatment is irreversible and if mistaken can lead to suicide. Diagnostic cues are essentially clinical and a test period of two years is recommended before surgery. During this period psychotherapy and antiandrogen and estrogen prescriptions are made to achieve secondary sexual characters adequacy. In this review article the concepts required for good practice in transsexualism are discussed as well as the therapeutic options available. O transexualismo como um fenômeno, claramente contrasta nossos entendimentos comuns sobre sexualidade (1,2), mas não é uma depravação sexual (3) e sim a forma mais extrema de distúrbio da identidade sexual (4). Também chamado de disforia de gênero (5), é uma incompatibilidade entre o sexo anatômico de um indivíduo e a sua identidade de gênero (6).Nestes indivíduos, o desejo de pertencer ao sexo oposto é extremamente forte, procurando desesperadamente a terapia hormonal e a cirurgia, com este objetivo (7,8).Conseqüentemente seu sofrimento é tanto e tão urgente que podem chegar ao extremo de auto-mutilação e suicídio (4).
