Examining the synergistic effects of a cognitive control video game and a home-based, self-administered non-invasive brain stimulation on alleviating depression: the DiSCoVeR trial protocol

Abstract: Enhanced behavioral interventions are gaining increasing interest as innovative treatment strategies for major depressive disorder (MDD). In this study protocol, we propose to examine the synergistic effects of a self-administered home-treatment, encompassing transcranial direct current stimulation (tDCS) along with a video game based training of attentional control. The study is designed as a two-arm, double-blind, randomized and placebo-controlled multi-center trial (ClinicalTrials.gov: NCT04953208). At thre… Show more

“…Another protocol describes a parallel-group RCT (double-active vs double-placebo), combining home-based tDCS and cognitive control training in the treatment of 114 MDD patients. In addition to investigating the synergy of tDCS and mini games for cognitive control training, an interesting aspect of this study is the implementation of an electrode-positioning algorithm that allows the correct positioning of the electrode cap even during self-application at home [ 21 ]. Another protocol describes a large RCT with 210 patients and three arms (70 each), where home-based tDCS is investigated in conjunction with internet-based behavioural therapy (iBT) [ 42 ].…”

“… The combination of tDCS with other interventions could also be tested in the home-based setting. This is expected to have a synergistic effect, feasibly improving the rather small effects of tDCS compared to placebo [ 21 ]. A RCT examining the combined effects of tDCS and behavioural therapy in a clinical group setting had negative results, however this study lacked a double-placebo condition [ 59 ].…”

“…The use of cognitive tasks with rTMS is more limited, but not impossible as demonstrated in recent studies. 37,38 On the other hand, rTMS produces effects of greater magnitude. We believe that the best results are obtained with NIBS associated with cognitive tasks during or after NIBS.…”

“…Antidepressant effects were promising and long lasting with 91.3% clinical response and 73.9% remission rates even in the 6-month follow up (28). In addition to the results of these pilot studies, case reports, and smaller monocentric single blinded trials, study protocols of larger RCTs on this topic even with the combination of tDCS with cognitive training or behavioural therapy have been published, though the results of these trials are expected in the future (29,30).…”
