Examination of Response Shift After Rehabilitation for Orthopedic Conditions: A Systematic Review

Abstract: Context: There is an increased emphasis on the need to capture and incorporate self-reported function to make clinical decisions when providing patient-centered care. Response shift (RS), or a change in an individual's self-evaluation of a construct, may affect the accurate assessment of change in self-reported function throughout the course of rehabilitation. A systematic review of this phenomenon may provide valuable information regarding the accuracy of self-reported function. Objectives: To systematically … Show more

“…14 Conservative care may not provide a substantial enough catalyst to prime individuals for an RS. 11 As such, our findings support using traditional preassessment-to-postassessment comparison methods to evaluate self-reported function after a conservative intervention for those with CAI. However, this should not be assumed for all treatment courses among patients with CAI.…”

“…Traditionally, RS has been observed in individuals with chronic, life-threatening conditions whose physical health deteriorates but whose self-reported HRQOL remains stable. 10 More recently, RS has gained attention as a possible phenomenon 11 after rotator cuff repair, 12 autologous chondrocyte implantation, 13 total knee arthroplasty, [14][15][16] knee microfracture, 17 lumbar spinal decompression surgery, 18 and unspecified rehabilitation for chronic low back pain, 19 all treatments for chronic musculoskeletal conditions. Response shifts in these populations can potentially affect the evaluation of the rehabilitation process and clinical decision making and lead to the underestimation or overestimation of patient change after treatment.…”

“…The acceptance of this methodology has been used in recent studies on RS applied to different clinical areas: oncology [ 23 , 28 ], orthopedic rehabilitation [ 29 ], pre-injury [ 30 ], as well as patient-reported outcomes (PRO) [ 31 ].…”

“…To our knowledge, there are only six systematic reviews on the study of RS, the topics of which include RS among cancer patients, RS after rehabilitation for orthopedic disorders, the clinical signi cance of RS, and RS detection methods, while none of them concerned patients with ESRD. [13][14][19][20][21][22]. This study is the rst and aimed to review the evidence, study design, detection methods, participants' characteristics, primary results, and conclusions of RS among ESRD patients.…”
