International Journal of Case Reports and Images (IJCRI) is an international, peer reviewed, monthly, open access, online journal, publishing high-quality, articles in all areas of basic medical sciences and clinical specialties.Aim of IJCRI is to encourage the publication of new information by providing a platform for reporting of unique, unusual and rare cases which enhance understanding of disease process, its diagnosis, management and clinico-pathologic correlations. We report a case of a 16-year-old female who presented at our department for one month history of pleuritic chest pain. The physical examination and the thoracic radiography results supported a pleural effusion. The results of the first thoracentesis and pleural biopsy were not conclusive, thus a second thoracentesis was attempted but did not withdraw any pleural fluid. A thoracoabdominal computed tomography (CT) scan was performed which found a large infraphrenic tumor suspecting an adrenal neuroblastoma. A CT guided biopsy was done and the histopathology study revealed a malignant proliferation made of medium sized round cells. The immunohistochemistry concluded to Ewing's sarcoma. The diagnosis of primitive adrenal gland neuroectodermal tumor was established and adjuvant chemotherapy was recommended. The patient passed away during her first chemotherapy session. IJCRI publishes Conclusion:The adrenal localization of Ewing's sarcoma is rare. A neuroblastoma is the first brought up diagnosis in such presentation but an extraskeletal neuroectodermal tumor is also possible.

HighlightsEwing sarcoma/primitive neuroectodermal tumor of the kidney (ES/PNET) is a member of Ewing’s sarcoma family, occurring in young adults.The clinical course and prognosis of ES/PNET are different from renal cell carcinoma (RCC).For definite diagnosis, in addition to cytogenetic analysis; other techniques may be needed; such as fluorescent in situ hybridization (FISH), reverse transcriptase-polymerase chain reaction (RT-PCR) of the t (11; 22) translocation [1].

Small Cell Variant of Renal Oncocytoma – a Case Report of Unusual Histopathologic Entity

Renal oncocytoma (RO) accounts for 3-7% of all renal cells tumors. It typically consistes of large eosinophilic cells (oncocytes) with abundant cytoplasm, which constitute the crucial diagnostic feature. In 2001, the Czeck authors first described an unusual small cell variant of RO and untill now, only a few reports of such cases have been published. In the current article, the author presents an additional new case. 40-year old male with macroscopic hematuria as a clinical symptom was diagnosed to have solitary tumor in the upper third of the right kidney. He underwent a nephrectomy. On light microscopy, the tumor was predominantly composed of uniform small cells ( " oncoblasts") with scant cytoplasm, hyperchromic nuclei and high nuclear-to-cytoplasmic ratio. In addition, it also contained characteristic oncocytes typical for oncocytoma. Tumor was strongly immunoreactive for EMA, sporadically positive for CK7 and negative for RCC antigen, vimentin, S100, WT1, chromogranin and synaptophysin. Proliferative activity did not exceed 1% and mitotic activity was virtually absent. No necrosis or aggressive growth features were found. The spectrum of histopathologic and immunohistochemical findings was consistent with a diagnosis of small cell variant of RO. The author focus on histopathological aspects and differential diagnostic pitfalls of this unique lesion.
