Evolving Trends in Surgical Management of Breast Cancer: An Analysis of 30 Years of Practice Changing Papers

Abstract: The management of breast cancer has evolved into a multidisciplinary evidence-based surgical speciality, with emphasis on conservative surgery. A number of landmark trials have established lumpectomy followed by radiation as the standard of care for many patients. The aim of this study is to construct a narrative review of recent developments in the surgical management of breast cancer and how such developments have impacted surgical practice. A comprehensive literature search of Pubmed was conducted. The late… Show more

“…In response to the constantly increasing cases of breast cancer, a huge effort has been made to encourage more frequent and easily accessible breast cancer screenings, as well as to find new and more effective diagnostic and treatment strategies [ 3 , 4 ]. Several treatments have been proposed for breast cancer [ 5 , 6 , 7 ], namely surgery [ 8 , 9 ], radiation [ 10 ] and/or chemotherapy [ 11 ], immunotherapy [ 7 ], hormone therapy [ 5 , 11 ] and targeted therapy [ 12 ]. Nevertheless, the outcomes of the currently available strategies are still typically associated with high impact both on patient’s life and self-esteem [ 13 ], and/or lack of specificity towards cancerous cells, which increases the chance of suffering from extensive adverse side effects [ 14 ].…”

“…Breast cancer is the most frequently diagnosed malignancy in women, with a lifetime risk of 12.4% in the western world [ 1 ]. While surgical resection through Halsted’s radical mastectomy was once considered the cornerstone of breast cancer management, novel therapeutic strategies and a more targeted approach to surgery have facilitated the improved oncological and survival outcomes, while minimizing treatment-related morbidity [ 2 ]. These timely changes to breast cancer care coincide with our heightened appreciation for the cellular, biomolecular, and genomic properties responsible for driving oncogenesis [ 3 , 4 ].…”

“…Historical data have shown that breast-conserving surgery followed by radiotherapy has equivalent oncological outcomes to those of mastectomy in early breast cancer ( 1 , 2 ). As long-term survival after breast cancer treatment has become commonplace, more attention has been given to develop oncoplastic techniques to provide better patient and aesthetic satisfaction ( 3 , 4 ). The primary role of oncoplastic breast-conserving surgery (OBCS) is to achieve oncological safety while minimizing the risk of unacceptable local deformity by allowing reconstruction of the defect and preventing the need for mastectomy ( 5 , 6 ).…”
