Evolving Minimally Invasive Techniques for Tear Trough Enhancement

Abstract: When the patient's primary concern is the appearance of the tear trough, and the physician is limited to treating either the tear trough or the cheek alone, it appears that treatment in the tear trough offers greater improvement in the appearance of the tear trough deformity when compared with treatment in the cheek alone.

“…Alloplastic materials such as hyaluronic acid are a good option when patients want procedures which are more practical [24] . Fat grafting is the author's preference due to many reasons: the fat is abundant, the procedure is autologous, fat grafting is a cell transplant which has anti-inflammatory "anti-fibrotic" and "trophic" effects with a possibility of achieving better skin quality, and the results are natural [25][26][27] .…”

“…Research focused on personal techniques purposed by the authors have been published since 2005. Airan and Born [11] and Kane [12] wrote the first articles ever about this topic, both in 2005; technique refinement and new trends have been published in 2006 [22,23] , 2007 [24,33] , 2011 [16,32,76] , 2012 [26,41,42,66] , 2013 [73] , 2014 [43] , 2015 [48][49][50] , 2016 [27,28] , 2017 [17,52] , 2018 [19,71,74] and 2019 [21] .…”

“…This can become more prominent as the maxillary bone regresses, soft tissues of the midface descend inferiorly and medially, and inferior orbital fat prolapses. 27 As the midface descends, the superior convexity lengthens vertically and creates a shadow inferiorly, causing patients to complain of "dark circles. “…Skin changes, including rhytids, photodamage, thinning, laxity, and a decrease in dermal collagen, elastin, and hyaluronic acid, are also a part of the facial aging process. 27,39 Optimal periorbital rejuvenation should treat these skin changes and not just the soft tissue and osseous changes. Various therapies have been described to stimulate dermal collagen synthesis and to improve the appearance of rhytids and dyschromia.…”
