Evolving Indications for and Trends in Keratoplasty in British Columbia, Canada, From 2002 to 2011

Abstract: FED has become the top indication for performing a keratoplasty over the 10-year period. There was a shift from PKP to DSAEK performed for endothelial failure. Although the number of deep anterior lamellar keratoplasty surgeries was small, there was a significant increasing trend.

“…This may, in part, explain why significantly more eyes were in stages 1 to 3 in the first compared with the second period. However, as discussed above, other authors have not reported a reduced frequency of keratoplasties in patients with keratoconus in recent years, [24][25][26] and one may assume that all these described treatment modalities were also available in their regions. It is likely that these treatments are offered mostly to patients with mild keratoconus in which keratoplasty is not considered as a treatment option.…”

“…3,5,14 However, thus far, studies on frequencies of keratoplasty for keratoconus have reported a steady level or even an increase in transplantations over the last 2 decades. [24][25][26] In 1 study, the authors claimed that there was a decrease in keratoplasties in patients with keratoconus, but this reduction was found only in the percentage relative to the total number of keratoplasties. The frequency of keratoplasties for keratoconus was actually stable: 93 and 91 in 2009 and 2013, respectively.…”

“…Beyond this global perspective of corneal blindness, our survey evaluated each country based on its own epidemiology. Indeed, corneal blindness varies worldwide [8][9][10] because of several factors: country's health and economic status, demographics, and environmental and geographic conditions. We identified about 185 000 CTs in 2012, the highest number ever reported.…”

“…International studies reported in the last few years corroborate this change abroad as well, with a shift away from PKP toward selective lamellar keratoplasties. 22,30,31 Our study is limited by several factors. The most significant limitation of our study is that our data were generated from a review of the surgical pathology database at WU.…”
