Evolution of surgical treatment for pharyngeal pouch

Abstract: Background: The development of endoscopic techniques, particularly endoscopic stapling, has led to a re-evaluation of the treatment of pharyngeal pouch. The pathophysiology and treatment of the condition is reviewed. Methods: An electronic literature search was undertaken on the pathophysiology, history and surgery of pharyngeal pouch (Zenker's diverticulum). These last two terms were used to the search the Cochrane, Medline and Embase databases (from 1966 to date) and the bibliographies of extracted articles. … Show more

“…Suspicion for an abnormal pouch leads to a CT scan of the neck and this subsequently aided surgical planning. Endoscopic diverticulotomy has been a well-accepted approach to surgical management of Zenker's diverticuli for the past two decades; however, Killian-Jamieson and Laimer's diverticula have usually been treated via an external approach [9, 10]. An external approach allowed for careful and accurate identification and dissection of the pouches, with preservation of the neurovascular bundle.…”

“…The great advantage compared to external approach is that endoscopic revision treatment is technically feasible and relatively easy, no more difficult than the primary endoscopic procedure, with no increase of risk. Moreover, patient satisfaction was higher in those who underwent endoscopic revision surgery compared to those who underwent open revision surgery [5, 12, 18]. …”
