Evolution and literature review of robotic general surgery resident training 2002–2018

Abstract: The University of Illinois purchased their first da Vinci System in September of 2002. Within the first calendar year, their program began orienting trainees to the da Vinci Standard System to make its inclusion in their clinical training run more smoothly. During the ensuring 16 years, their program has evolved into more frequent resident orientations, lectures, and courses. The program has grown over the course of different versions of the da Vinci System. Currently, their program houses three Xi and two Si … Show more

“…6 Over the ensuing years, the program evolved along with the technology, aided in particular by the acquisition of the telestrator in 2009 and the dual surgeon's console in 2012. 7 Yet, the need persisted for a more structured training program that could track participant performance over time, leading to the establishment of a formal curriculum in June 2017. Studying the residents' progress through this new curriculum has been approved and monitored by an Institutional Review Board.…”

“…Consecutively, an overall experience score was determined by assigning 'experience score count' (ESC) from 0 to 3 to each experience level. Using this scoring system, the overall experience score varies between 0 and 9, and this results in three levels of surgical practical competency (surgical experience level): basic (0-3), advanced (4-5) and expert (6)(7)(8)(9).…”

“…Nevertheless, the procedures will become more and more an integral part of daily routine [ 5 ]. Therefore, it is emendable to standardize training concepts [ 1 ] and refer them to the education of unexperienced surgeons during their residency (so called “young” surgeons) [ 6 , 7 ]. Development of a feasible curricular structure which can be implemented into training programs of other facilities is a challenge referring to change management as an important and ongoing process [ 8 , 9 ].…”

“…Schlussendlich muss man konstatieren, dass dies wohl nicht an allen Kliniken möglich und wenn überhaupt auf 1-2 Organbereiche pro Klinik begrenzt sein wird. [8]. Auffällig ist hierbei eine offensichtliche Anlehnung an Ausbildungsprogramme der Aviatik, die sich nicht nur auf operationstechnische Aspekte konzentrieren, sondern darüber hinaus auch den Fokus auf Entscheidungsfindungen und Notfallsituationen legen [9].…”
