Evidence of pudendal neuropathy in patients with perineal descent and chronic straining at stool.

Abstract: SUMMARY In 17 women with chronic constipation, and abnormal perineal descent on straining at stool, there was more severe neurogenic damage to the external anal sphincter muscle and to its pudendal innervation in those patients with a long history than in those with a short history. These results suggest that recurrent trauma to the pudendal nerves can occur during perineal descent, and that this can lead to denervation and weakness of the external anal sphincter muscle.Between 30% and 66% of patients with fae… Show more

“…Constipation has been reported to be the predominant symp tom in 19-65% of patients [26][27][28], Fecal incontinence is also commonly associated with rectal prolapse [29] and is a common long-term complication. This may be due to a combination of repeated traction on the pu dendal nerves, abnormal perineal descent and progressive anal dilatation [16,22,30], Pro gressive anal dilatation is thought to lead to low resting [31] and squeeze pressures [32,33]. Internal anal sphincter dysfunction is the result of chronic stretching, activation of the rectoanal inhibitory reflex [34] and disturbed anorectal sensation [35], External anal sphincter weakness, on the other hand, mani fested by reduced squeeze pressure, is the result of denervation [32,33].…”
